A inflamação das vias respiratórias possui um papel central na sua patogénese, contudo os mecanismos envolvidos não estão ainda totalmente esclarecidos. Com base nas novas técnicas independentes de cultura, atualmente é reconhecida a existência de alterações na composição e abundância do microbioma das vias aéreas de doentes com patologia pulmonar, particularmente na DPOC. Porém, está ainda por clarificar se a ocorrência de disbiose é causa ou consequência das transformações fisiopatológicas subjacentes à doença e de que forma pode influenciar a progressão desta.Nesta revisão, fornece-se uma perspetiva sobre os métodos de estudo do microbioma respiratório, sobre a sua composição e sobre as modificações que nele ocorrem, no âmbito da DPOC. É também discutido o efeito dos regimes terapêuticos e de que forma modulam a microbiota respiratória na doença estável e nas exacerbações. Finalmente, são descritas as limitações dos estudos atuais e as perspetivas futuras de investigação. . Chronic Obstructive Pulmonary Disease (COPD) is a complex, preventable and treatable disease, that represents a leading cause of morbimortality worldwide. Inflammation of the respiratory tract plays a central role in its pathogenesis, though the mechanisms involved are not yet fully understood. Based on the new culture-independent techniques, nowadays it is acknowledged the existence of changes in the composition and abundance of the respiratory tract microbiome of patients with pulmonary diseases, particularly COPD. However, it remains to be clarified whether the occurrence of dysbiosis is a cause or a consequence of the pathophysiological changes implied in the disease and how it can influence its progression.In this review, a perspective is given on the methods of studying the respiratory microbiome, on its composition and on the changes that occur in it, regarding COPD. It is also discussed the effects of therapeutic regimens and how they modulate the respiratory microbiota in stable disease and in exacerbations. Finally, the limitations of current studies and future perspectives of investigation are described. . |
