Teve duração de quatrocentas horas e decorreu em contexto de internamento e de consulta externa. Objectivos: Os principais objetivos para o estágio foram aprender e aplicar corretamente a avaliação geriátrica global e as escalas de avaliação geriátrica; trabalhar as capacidades de diagnóstico e intervenção para as principais síndromes geriátricas e de gestão da multimorbilidade; implementar plano de intervenção multidisciplinar para prevenção, tratamento, manutenção ou reabilitação do doente geriátrico. Métodos: As actividades desenvolvidas consistiram no acompanhamento diário de doentes internados na unidade de Ortogeriatria, no acompanhamento dos doentes vigiados em consulta de geriatria e participação nas reuniões de equipa. Resultados: O principal motivo de internamento dos doentes acompanhados foi fractura da extremidade proximal do fémur. Os doentes mantinham acompanhamento na consulta após a alta do internamento. Foi praticada a realização da avaliação geriátrica global, aplicação de escalas de avaliação geriátrica, construção de planos de cuidados individualizados e integrados, bem como intervenção na vertente da desprescrição. Conclusão: No final do estágio considera-se que as actividades realizadas ao longo do tempo de estágio permitiram que todos os objectivos inicialmente propostos foram cumpridos. Abstract: Introduction: This activity report describes the events carried out in the practical internship in geriatrics at an Orthopedics unit in a hospital in the metropolitan area of Lisbon, Portugal. It lasted four hundred hours. The intenship took place in the othogeriatric inpatient ward and external appointments. Objectives: The main objectives for the internship were to learn and correctly apply the global geriatric assessment; to learn and correctly apply the geriatric assessment scales; work on diagnostic skills and intervention skills for the main geriatric syndromes; work on multimorbidity management; to learn and correctly implement a multidisciplinary intervention plan for the prevention, treatment, maintenance or rehabilitation of the geriatric patient. Methods: The activities carried out were daily monitoring of patients admitted to the Orthopedics unit, monitoring patients in the outpatient setting and participating in team meetings were debriefings were made. Results: The most common reason for hospital stay was proximal femoral fracture. Patients were followed up at the appointments after discharge from the hospital. Global geriatric assessment was carried out, geriatric assessment scales were applied, individualized and integrated care plans were made, as well as description protocols. Conclusion: At the end of the internship, it is considered that the activities carried out during the internship allowed that all the objectives initially proposed were fulfilled. |
