Abstract: Introdução: A multimorbilidade é um dos temas mais analisados no âmbito da investigação da MGF. A relevância do seu estudo prende-se com o acentuado envelhecimento da população mundial e da sua longevidade, bem como com a circunstância dos doentes com multimorbilidade constituírem uma parte substancial dos pacientes dos médicos de MGF. Todavia, a investigação sobre a multimorbilidade tem-se centrado quase exclusivamente sobre os doentes. São praticamente inexistentes os estudos que examinaram a perceção dos médicos sobre a sobrecarga da gestão da multimorbilidade em MGF. Associada a esta realidade, está também o facto de não existirem em Portugal instrumentos adequados e validados para avaliar a sobrecarga da gestão da multimorbilidade. A qualidade de vida dos médicos está associada a efeitos negativos no cuidado dos pacientes. Dada a escassez de investigações sobre estes temas, pareceu-nos pertinente examinar o impacto da sobrecarga da gestão da multimorbilidade nos médicos de MFG na sua QdVRS.Objetivos: Procurámos validar um instrumento de avaliação da sobrecarga da gestão da multimorbilidade em MGF (SoGeMM-MGF). Pretendíamos analisar o impacto da gestão da multimorbilidade nos médicos de MGF na sua QdVRS. Tentámos verificar a influência de algumas variáveis sociodemográficas e profissionais na gestão da multimorbilidade e na QdVRS desses médicos.Métodos: O estudo foi realizado em duas fases. Na primeira efetuou-se uma análise sobre as caraterísticas psicométricas do SoGeMM-MGF, tendo participado na mesma 32 médicos de MGF da ARS Centro de Portugal. Na segunda fase estiveram envolvidos 221 médicos inscritos como especialistas em MGF na Secção Regional do Centro da Ordem dos Médicos. Foi administrado aos médicos o SoGeMM-MGF, no sentido de se examinar a perceção de sobrecarga da gestão da multimorbilidade, e o SF-12 para se avaliar a sua QdVRS. Na validação do SoGeMM-MGF, procedeu-se a uma análise fatorial exploratória. Recorreu-se ao coeficiente de correlação de Spearman para analisar as associações entre variáveis e aos testes de Mann-Whitney e Kruskal-Wallis para a comparação de grupos.Resultados: Na validação do SoGeMM-MGF, a análise da fidelidade revelou um alfa de Cronbach de 0,815 e a correlação entre as pontuações totais do teste-reteste foi de r= 0,854. A estrutura do questionário organiza-se de forma consistente em cinco fatores que explicam 59,54% da variância dos resultados nos itens. Registou-se uma pontuação média no SoGeMM-MGF de 79,81±6,513 e nos domínios Physical Component Summary (PCS) e Mental Component Summary (MCS) do SF-12 de 47,974±9,289 e 44,606±11,025, respetivamente. Verificou-se que existe uma correlação negativa e estatisticamente significativa entre as pontuações obtidas no SoGeMM-MGF e o MCS do SF-12 (rs= -0,267; p= 0,038). Foram encontradas diferenças significativas nas pontuações obtidas pelos médicos no SoGeMM-MGF em função das variáveis: tempo de trabalho com doentes com multimorbilidade, número de consultas semanais com estes doentes, proporção de doentes com multimorbilidade que acompanham. Ao invés, somente o efeito da variável sexo suscita diferenças significativas na QdVRS dos médicos de MGF, particularmente no domínio da saúde mental (U= 251,50; p= 0,021), apresentando os médicos do sexo masculino pontuações mais elevadas no MCS. Discussão e Conclusões: Os coeficientes de fidelidade e a estrutura fatorial do SoGeMM-MGF parecem satisfatórios de forma a possibilitarem a sua utilização em estudos posteriores realizados em Portugal sobre a gestão da multimorbilidade nos médicos de MGF. Constatou-se que diversas variáveis profissionais dos médicos de MGF influenciavam a avaliação que fazem da sobrecarga da gestão da multimorbilidade. Em contrapartida, os dados sugeriram a não verificação de efeitos significativos das variáveis profissionais nos domínios da saúde física e mental dos médicos de MGF. O estudo mostra que quanto maior é a avaliação da sobrecarga da gestão da multimorbilidade por parte dos médicos de MGF, mais negativa é a perceção do seu estado de saúde mental. De salientar que a relação observada entre estas duas variáveis pode representar um desafio para a forma como os cuidados de saúde primários são organizados e prestados. Devido ao aumento da prevalência da multimorbilidade na população e ao efeito que a gestão destes doentes tem na saúde mental dos médicos, se não existirem estratégias para a reorganização dos cuidados de saúde primários destes doentes poder-se-á, no futuro, acentuar a deterioração da qualidade do seu atendimento. Este foi um dos primeiros estudos que analisou em Portugal a relação entre a perceção da sobrecarga da gestão da multimorbilidade dos médicos de MGF e a sua QdVRS, sendo necessários mais estudos sobre esta temática, particularmente com amostras mais amplas e que abranjam todo o território nacional.
Introduction: Within primary care research, multimorbidity is one of the most analyzed themes. The relevance of multimorbidity study research is grounded on the pronounced aging of the world population and its longevity, as well as on the fact that patients with multimorbidity constitute a substantial part of primary care physicians’ patients. However, research on multimorbidity has been almost exclusively focused on patients. There are almost no studies that have examined-physicians' perceptions about the work overload of multimorbidity management in primary care. Associated with this reality, there are no adequate and validated instruments in Portugal to assess the work overload of multi-morbility management. The physicians’ quality of life has been associated with negative effects on patients care. Given the fact research within this line of inquiry is still scarce, it seemed relevant to examine the impact of the work overload of multimorbity management on primary care physicians and in their HRQoL.Objectives: We sought to validate an instrument to assess the impact of work overload of multimorbidity management in primary care (SoGeMM-MGF). We aimed to analyze the impact of multimorbidity management on primary care physicians in their HRQoL. We attempted to verify the influence of some sociodemographic and professional variables in the management of the multimorbidity and in the HRQoL of these physicians.Methods: The study was carried out in two phases. In the first one, an analysis was performed on the psychometric characteristics of SoGeMM-MGF and included 32 primary care physicians from ARS Centro de Portugal. In the second phase, 221 physicians registered as primary care physicians in Center Regional Section of the Medical Order were involved. SoGeMM-MGF was administered to physicians in order to examine their perceptions on the overload of multitimorability management, and SF-12 to assess their HRQoL. In the validation of the SoGeMM-MGF, an exploratory factorial analysis was. Spearman's correlation coefficient was used to analyze the associations between variables and the Mann-Whitney and Kruskal-Wallis tests for group comparison purposes.Results: In SoGeMM-MGF validation, the fidelity analysis revealed an alpha Cronbach of 0.815 and the correlation between total test-retest scores was r = 0.854. The questionnaire structure is consistently organized in five factors that explain 59.54% of the variance within the results in each of the items. The analysis showed a mean score in the SoGeMM-MGF of 79.81 ± 6.513 and a mean score of 47.974 ± 9.289 and 44.606 ± 11.025 in the SF-12 PCS and MCS domains respectively. We verified there is a negative and statistically significant correlation between the scores obtained in SoGeMM-MGF and the MCS of SF-12 (rs = -0.267; p≤ 0,038). Significant differences were found in scores obtained by physicians in SoGeMM-MGF, depending on these variables: working time with patients with multimorbidity, number of weekly consultations with these patients, proportion of patients with multimorbidity. On the other hand, only the effect of the gender variable raised significant differences in the HRQoL of primary care physicians, especially in mental health (U = 251.50; p < 0,021), with male physicians presenting higher MCS scores.Discussion and Conclusions: The fidelity coefficients and the factorial structure of the SoGeMM-MGF seem satisfactory enough to allow its use in future studies carried out in Portugal on management of multimorbility of primary care physicians. The findings showed that several professional variables of the primary care physicians influenced the evaluation they make of the overload of multimorbidity management. On the other hand, the findings suggest there are not significant effects of the professional variables in the physical and mental health of primary care physicians. The present study shows that a greater assessment of multimorbity management work overload by primary care physicians, is associated to more negative perceptions on their mental health status. It should be noted that the observed relationship between these two variables can present a challenge to the way primary care is organized and provided. Due to the increasing prevalence of multimorbidity in the general population and the effect managing these patients has on physicians' mental health, if there are no strategies for reorganizing primary health care provided to these patients, there may be a further deterioration in the quality of their service. In Portugal, this was one of the first studies that analyzed the relationship between the perceived overload of primary care physicians' multimorbidity management and their HRQoL. Future studies within this line of inquiry are needed, particularly with larger samples covering the entire national territory.
