Estatinas e Diabetes: "friends or foes"

Abstract

Within the scope of the unit of Curricular Internship of the Integrated Master’s degreein Pharmaceutical Sciences of the Faculty of Pharmacy of the University of Coimbra, thisdocument presents, in the form of a SWOT analysis (Strengths, Weaknesses, Opportunitiesand Threats), the internship report in Community Pharmacy, performed at Farmácia Nazareth,in Coimbra, from 7th of January to 29th of April 2019, under the guidance of Dr. Ana PatríciaDavid and the internship report at the Health Technology Assessment Department ofINFARMED - National Authority of Medicines and Health Products, I.P, in Lisbon, from 7th ofMay to 31st of July 2019, under guidance of Professor Cláudia Furtado and pharmacist SaraCouto. This document also includes the monography entitled "Statins and Diabetes: friends orfoes”.Statins are HMG-CoA reductase inhibitors, which reduce the levels of LDL cholesterolwith few side effects. Indeed, these drugs are very beneficial for people who suffer fromcardiovascular diseases, by reducing the morbidity and mortality risk, which consequently ledto its indiscriminate use all over the world. However, over the past few years, new informationhas arisen about the association between these drugs and the development of diabetesmellitus. Several observational studies have been conducted to investigate this association andalso some in vitro studies, in order to discover the mechanisms of action that are triggered bystatins and can lead to the development of diabetes. Thus, this monography focuses on therole of statins in diabetes mellitus, counterbalancing its positive and negative effects on patientswith this disease and their mechanisms of action, including an initial contextualization of this drugs.

Ana Patrícia David e o relatório de estágio na Direção de Avaliação de Tecnologias deSaúde (DATS) do INFARMED - Autoridade Nacional do Medicamento e Produtos de Saúde,I.P., em Lisboa, com início a 7 de maio e término a 31 de julho de 2019, sob orientação daProfessora Cláudia Furtado e farmacêutica Sara Couto. Este documento inclui ainda amonografia intitulada “Estatinas e Diabetes: friends ou foes”.As estatinas são fármacos inibidores da redutase da HMG-CoA, reduzindo os valores decolesterol LDL e com poucos efeitos adversos associados. De facto, estes fármacos são umamais-valia nos doentes que sofrem de doenças cardiovasculares, reduzindo o risco demorbilidade e mortalidade, o que levou consequentemente ao seu uso indiscriminado portodo o mundo. Contudo, ao longo dos últimos anos, têm surgido novas informações sobre aassociação entre a toma destes fármacos e o desenvolvimento de Diabetes Mellitus. Váriosestudos observacionais foram realizados para averiguar esta associação e também algunsestudos in vitro, de modo a descobrir os mecanismos de ação que se desencadeiam com atoma da estatina, que podem levar ao desenvolvimento de diabetes. Assim, a presentemonografia foca o papel das estatinas na Diabetes Mellitus, contrabalanço os seus efeitospositivos e negativos nos doentes que sofrem desta patologia e os seus mecanismos de ação,incluindo ainda uma contextualização inicial sobre os fármacos em questão.
