About 46% of the new drug approvals by the Food and Drug Administration (FDA) are for oral delivery and more than 70% of the new chemical entities (NEQ) contained in the Pharmaceutical Industry pipeline are poorly water-soluble. Oral absorption requires the presence of the drug molecules dissolved in the gastrointestinal fluids. However, if they exhibit a good absorption but the dissolution rate is significantly slower, then the dissolution will translate into a limiting step of absorption and consequently the drug will present a low bioavailability.One of the biggest challenges of the Pharmaceutical Industry is to develop strategies that increases the solubility of poorly soluble drugs in water, this means hydrophobic drugs classified as Class II (ex: poorly soluble/very permeable) or Class IV (ex: poorly soluble/poorly permeable) according to the Biopharmaceutical Classification System.There are several ways developed and explored by the Pharmaceutical Industry in order to increase the solubility of drugs poorly soluble in water that will be initially addressed, but that are falling into disuse due to the investment and the use of new innovative techniques.Currently there are many studies that demonstrate the high potential of the use of cyclodextrins, polymeric micelles, lipid nanoparticles and liposomes as vectors of drugs practically insoluble in water in chronic diseases such as Diabetes Mellitus, Rheumatoid Arthritis and Schistosomiasis where the need for innovative techniques is urgent. That way, an approach will be taken to the promising use of these drugs carriers, which seek to achieve the maximum therapeutic efficacy. A administração oral de fármacos é considerada a melhor via de administração de fármacos para alcançar efeitos terapêuticos e profiláticos contra várias doenças, especialmente em tratamentos de longa duração, devido à sua simplicidade, tolerabilidade, aceitabilidade, e baixo custo. Cerca de 46% das novas moléculas aprovadas pela Food and Drug Administration destinavam-se à administração por via oral e mais de 70% das novas entidades químicas que constam na pipeline da Indústria Farmacêutica são consideradas pouco solúveis.Para que ocorra a absorção oral de um fármaco, é necessário que este esteja dissolvido nos fluídos gastrointestinais, e se estes apresentarem uma boa absorção mas a velocidade de dissolução for significativamente mais lenta, então a dissolução traduz-se num passo limitante da absorção e, consequentemente, o fármaco apresentará baixa biodisponibilidade. Um dos maiores desafios que a Indústria Farmacêutica enfrenta, é desenvolver estratégias que aumentem a solubilidade de fármacos pouco solúveis em água, ou seja, de fármacos hidrofóbicos, classificados como Classe II (i.e., pouco solúveis/muito permeáveis) ou Classe IV (i.e., pouco solúveis/pouco permeáveis) de acordo com o Sistema de Classificação Biofarmacêutico.Existem vários métodos desenvolvidos e explorados pela Indústria Farmacêutica, de modo a aumentarem a solubilidade de fármacos pouco solúveis em água, que irão ser abordados inicialmente, mas que têm vindo a cair em desuso, devido à utilização e investimento de novas técnicas inovadoras. Atualmente, vários estudos demonstram o elevado potencial da utilização de ciclodextrinas, micelas poliméricas, nanopartículas lipídicas e lipossomas, como vetores de fármacos praticamente insolúveis em água, em doenças crónicas como Diabetes Mellitus, Artrite Reumatóide e Esquistossomose, onde urge a necessidade de técnicas inovadoras. Deste modo, será realizada uma abordagem ao uso promissor destes transportadores de fármacos, que visam alcançar a máxima eficácia terapêutica. |
