Abstract: Introduction: Cleft Lip and Palate is a congenital anomaly with a high prevalence and is considered by the World Health Organization a public health problem. It is responsible for several anatomical changes that can lead to a decrease in the volume of the upper airways. There is an increased risk of respiratory complications resulting, in some cases, from this volumetric decrease. This anomaly requires a multidisciplinary treatment from birth to adulthood. The orthodontic treatment comprises, in most cases, the maxillary expansion that aims at the correction of the characteristic transverse and sagittal maxillary collapse of these patients.Objective: Assuming that the maxillary expansion, necessary for the correction of transverse maxillary deficiency, promotes changes in the anatomy of upper airways, this study intended to determine, from Cone Beam Computed Tomography, changes in upper airways volume in patients undergoing maxillary expansion. Materials and Methods: The upper airway volume of 19 patients (12 males and 7 females) with Cleft Lip and Palate was measured by the three-dimensional analysis program InVivo 6 in Cone Beam Computed Tomographies taken before and after maxillary expansion. The data was later statistically treated by the IBM SPSS Statistics program.Results/Discussion: In the nasopharynx, contrary to the expected, no statistically significant volumetric differences were found between T0 and T1. Also in the oropharynx and in the hypopharynx, no statistically significant volumetric differences were obtained between T0 and T1. This result coincides, with respect to the oropharynx, with the results obtained in most of the previous studies. There are no studies in this field regarding the hypopharynx.Conclusions: There were no statistically significant differences in the volume of the various upper airway regions (nasopharynx, oropharynx and hypopharynx) before (T0) and after (T1) maxillary expansion.
Introdução: A Fenda Lábio-Palatina é uma anomalia congénita de elevada prevalência, considerada pela Organização Mundial de Saúde um problema de saúde pública. É responsável por diversas alterações anatómicas faciais que podem levar, nomeadamente, à diminuição do volume das vias aéreas superiores. Verifica-se um aumento do risco de complicações respiratórias decorrentes, em certos casos, dessa diminuição volumétrica. Esta anomalia requer um tratamento multidisciplinar que decorre desde o nascimento até à idade adulta. A componente ortodôntica compreende, na maior parte dos casos, a expansão maxilar que visa a correção do colapso transversal e sagital maxilar característico nestes doentes. Objetivo: Partindo do princípio que a expansão maxilar necessária à correção da endognatia maxilar promove acessoriamente alterações ao nível das vias aéreas superiores, neste estudo pretendeu-se determinar, com recurso à análise de tomografias computorizadas de feixe cónico, as alterações de volume destas vias em doentes sujeitos a expansão maxilar.Materiais e Métodos: Em tomografias computorizadas de feixe cónico foi medido, através do programa de análise a três dimensões InVivo 6, o volume das vias aéreas superiores, antes (T0) e após (T1) expansão maxilar, de 19 doentes portadores de fenda lábio-palatina (12 do sexo masculino e 7 do sexo feminino), com idade média no início da expansão maxilar de 10,9 anos. Os dados obtidos foram posteriormente sujeitos a tratamento estatístico na plataforma estatística IBM SPSS.Resultados/Discussão: Ao nível da nasofaringe não foram encontradas diferenças volumétricas estatisticamente significativas entre T0 e T1, facto não concordante com estudos prévios em doentes não portadores da patologia. Também ao nível da orofaringe e da hipofaringe não foram encontradas diferenças volumétricas estatisticamente significativas entre T0 e T1. Estes resultados coincidem, relativamente à orofaringe, com a maior parte dos estudos prévios. Não existem estudos neste âmbito no que diz respeito à hipofaringe. Conclusões: Não foram verificadas diferenças estatisticamente significativas no volume das diversas regiões das vias aéreas superiores (nasofaringe, orofaringe e hipofaringe) antes (T0) e após (T1) a expansão maxilar.
