Abstract: O síndrome nefrótico pediátrico é uma doença glomerular que tem associada graves sintomas e comorbilidades. A origem de algumas destas comorbilidades está na terapêutica utilizada e no facto de algumas destas crianças apresentarem resistência à mesma. Com esta revisão da literatura pretendeu-se perceber que mecanismos estão associados à resistência a corticóides, bem como apurar que mecanismos estão sob investigação no sentido de ultrapassar esta resistência. Para obtenção de literatura pertinente foi feita uma pesquisa, em bases de dados médicas, de estudos realizados entre 2006 e 2017. Existem vários mecanismos para a resistência existente. Alterações nos níveis dos GR têm sido a principal linha de investigação, sendo que estes se encontram diminuídos no SRNS, embora este resultado nem sempre tenha sido unanime. Recentemente, estas alterações têm-se verificado nas células renais, para além de estarem presentes nas células imunitárias. Outras vias investigadas foram o TNF-α, o HSP90 e o MIF, todos apresentando alterações na presença de SRNS. As vias genéticas apuradas não apresentaram relação com a resistência. Novas terapêuticas foram investigadas para contornar esta resistência. A Pioglitazona, um antidiabético, permitiu obter níveis iguais de redução de proteinúria com níveis inferiores de corticóides. Anticorpos anti-TNF-α permitiram regularizar os níveis de GR dos doentes. Também foram procuradas formas de predizer a resposta à corticoterapia, com base em alguns indicadores, para tentar diminuir comorbilidades. Concluindo, a raridade da doença leva a dificuldades na investigação mas, ainda assim, é importante continuar a investigar novos mecanismos e melhorar o entendimento existente desta patologia para aumentar a qualidade de vida dos doentes.
Paediatric nephrotic syndrome is a glomerular disease associated with severe symptoms and comorbidities. The origin of some of these comorbidities is in the therapeutic and in the fact that some of these children present resistance to the treatment. With this revision of literature we tried to understand what mechanisms are associated with glucocorticoid resistance, as well as what mechanisms are under investigation as ways to surpass this resistance. To obtain relevant literature we did a search, in medical databases, of articles from the years between 2006 and 2017. There are various mechanisms for the existence of resistance. Changes in the level of GR have been the principal line of investigation, being found reduced in SRNS, even if this result has not always been unanimous. Recently, these alterations were found in renal cells instead of only immunity system cells. Other pathways investigated were TNF-α, HSP90 and MIF, all of them presenting changes in SRNS. Genetic pathways found did not present relation with resistance. New therapeutics have been investigated to bypass this resistance. Pioglitazone, a type II diabetes drugs, has allowed an equal reduction of the levels of proteinuria with inferior levels of glucocorticoids. TNF-α antibodies righted the levels of GR in patients. Ways to predict the response to glucocorticoids based on some indicators were also searched as a method to reduce comorbidities. To conclude, the rarity of the disease led to difficulties in the investigation but, nonetheless, it is important to continue to research new mechanisms to better understand this pathology so that we can improve the quality of life of the patients.
