Abstract: Introduction: The importance of physical activity is increasingly recognized. There is scientific evidence that physical activity in healthy pregnancy is safe and desirable and should be encouraged by health professionals who are duly informed about its benefits showed in maternal and child health. Objective: Literature revision in order to systematize the health benefits of physical activity in pregnancy, to enable proper and safe promotion of its practice by health professionals. Methods: The narrative review comprised extensive bibliographical research in PubMED and Cochrane databases, on May 22nd 2018, of articles related to physical activity and benefits of pregnancy, with the MESH terms: physical activity, exercise, pregnancy, pregnant woman and not MESH: benefits. Articles published in the last 5 years were included, written in Portuguese, English and Spanish, resulting in 161 articles, which after selection of those considered relevant based on title, abstract and integral reading, resulted in 28 articles. All the bibliographic references were analyzed to add relevant and current additional articles.ResultsPhysical activity during pregnancy results in multiple benefits for maternal and child health, which can influence the well-being of both. However, most of the results are inconsistent, respectively regarding maternal health benefits in terms of hemodynamic, cardiovascular benefits, prevention of preeclampsia and gestational diabetes, improvement of depressive symptoms, reduction of cesarean deliveries and labor duration. Better physical fitness and perceived quality of life of women, the improvement of glucose tolerance and insulin sensitivity in pregnancy with gestational diabetes, the modulation of gestational weight gain, the reduction of musculoskeletal pain and advantages known in the obese pregnant woman, seem to have more evidence that is more robust. There is also the potential benefit for the fetus, however, although the data is still controversial, some studies refer to a possible reduction in the number of fetal macrosomia, with no risk increase of premature childbirth and benefits in terms of the autonomic nervous system and neuro-behavioral maturation. Conclusion: This revision systematizes the evidence that physical activity is safe and brings important benefits for healthy pregnant women and their offspring and should be recommended and prescribed consciously by health professionals, in the absence of contraindications. There is need of future investigations, carrying out homogeneous studies of large size and considering the FITT recommendations (frequency, intensity, time and type) for the physical exercise programs, in order to explore and to prove maternal and fetal health benefits.
Introdução: A importância da atividade física é cada vez mais reconhecida. Há evidência científica de que a atividade física na gravidez saudável é segura e desejável, devendo ser incentivada por parte dos profissionais de saúde, devidamente informados acerca dos seus benefícios demonstrados na saúde materna e infantil. Objetivo: Revisão da literatura por forma a sistematizar os benefícios para a saúde da atividade física na gravidez, para permitir a promoção adequada e segura da sua prática por parte dos profissionais de saúde. Métodos: A revisão narrativa consistiu numa pesquisa bibliográfica alargada na base de dados PubMED e Cochrane, a 22 de maio de 2018, de artigos relacionados com a atividade física e os benefícios na gravidez, com os termos MESH: physical activity (atividade física), exercise (exercício físico), pregnancy (gravidez), pregnant woman (grávida) e não MESH: benefits (benefícios). Foram incluídos artigos publicados nos últimos 5 anos, escritos em Português, Inglês e Espanhol, tendo resultado um total de 161 artigos, que após seleção dos artigos considerados relevantes com base no título, abstract e leitura integral, resultou num total de 28 artigos. A lista de referências bibliográficas dos estudos citados foi analisada de modo a acrescentar artigos adicionais relevantes e atuais. Resultados: A atividade física durante a gravidez resulta em múltiplos benefícios para a saúde materna e infantil, que podem influenciar o bem-estar de ambos. No entanto, a maioria dos resultados são inconsistentes, nomeadamente no que respeita aos benefícios para a saúde materna em termos de benefícios hemodinâmicos, cardiovasculares, na prevenção da pré-eclâmpsia e diabetes gestacional, na melhoria dos sintomas depressivos, na redução da incidência de partos por cesariana e na duração do trabalho de parto. A melhoria da aptidão física e qualidade de vida percebida da mulher, a melhoria da tolerância à glicose e da sensibilidade à insulina em grávidas com diabetes gestacional, a modulação do ganho de peso gestacional, a redução da dor músculo-esquelética e vantagens conhecidas na mulher grávida obesa, parecem ter evidência mais robusta. Existe potencial benefício também para o feto, contudo, apesar dos dados serem ainda controversos, alguns estudos referem uma possível redução do número de recém-nascidos macrossómicos, sem aumento do risco de parto prematuro e benefícios em termos do sistema nervoso autonómico e maturação neuro-comportamental. Conclusão: A presente revisão estrutura a evidência de que a atividade física é segura e conduz benefícios importantes para as mulheres grávidas saudáveis e para a sua descendência, devendo ser recomendada e prescrita conscientemente pelos profissionais de saúde, na ausência de contraindicações. Existe a necessidade de investigações futuras, realizando estudos mais homogéneos, de maiores dimensões e ponderando as recomendações FITT (frequência, intensidade, tempo e tipo) para os programas do exercício físico, de forma a explorar e a comprovar os benefícios maternos e fetais para a saúde.
