Ainda que algumas destas condições sejam benignas, é importante identificar os seus portadores, pois estes podem apresentar susceptibilidade aumentada para intoxicação por determinadas substâncias. Por outro lado, a Síndroma de Crigler-Najjar pode originar manifestações neurológicas irreversíveis, como o kernicterus, cuja incidência tem vindo a aumentar.
A hiperbilirrubinémia isolada é a única manifestação laboratorial das formas congénitas familiares não hemolíticas.
Apesar da maioria destas formas congénitas serem pouco prevalentes, o seu conhecimento é indispensável para o diagnóstico diferencial da hiperbilirrubinémia isolada.
Desta forma, os objectivos primordiais desta revisão passam por avaliar o valor semiológico da hiperbilirrubinémia isolada e os efeitos que lhe estão associados; analisar e compreender os mecanismos fisiopatológicos que lhe são inerentes; sistematizar e classificar as hiperbilirrubinémias familiares congénitas não hemolíticas; identificar os meios de diagnóstico passíveis de esclarecer a sua etiologia; efectuar adequadamente o diagnóstico diferencial de hiperbilirrubinémia isolada e, por fim, desenhar as estratégias terapêuticas mais convenientes The study of congenital familial nonhemolytic hyperbilirubinaemias is important, because it includes not only benign pathologies, such as Gilbert Syndrome, but also forms with a high mortality rate, such as Crigler-Najjar Syndrome. Even if some conditions are benign, it is important to identify them, because these patients show increased susceptibility to certain drugs. On the other hand, Crigler-Najjar Syndrome can cause irreversible neurological manifestations, such as kernicterus, whose incidence has been increasing. Isolated hyperbilirubinaemia is the only laboratory manifestation of these nonhemolytic familiar congenital forms. Although most of these congenital forms are not prevalent, their knowledge is essential for the differential diagnosis of isolated hyperbilirubinaemia. Thus, the primary objectives of this review are to evaluate the semiological value of isolated hyperbilirubinaemia and its effects; to analyze and understand its pathophysiological mechanisms; to systematize and classify the familiar congenital nonhemolytic hyperbilirubinaemias; to identify diagnostic means that can help clarify its etiology; to correctly do the differential diagnosis of isolated hyperbilirubinaemia, and finally to draw the most appropriate therapeutic strategies |
