Asma grave em adultos

Abstract

Este trabalho tem por objectivo oferecer informação plena e fiável acerca da asma grave, dos seus fenótipos e novas bordagens terapêuticas, com especial interesse sobre a terapêutica biológica emergente com omalizumab, bem como fazer uma breve referência à epidemiologia da asma e ao diagnóstico do doente asmático difícil de controlar. A realização deste trabalho teve por base uma revisão actualizada da literatura, por forma a criar uma nova fonte literária de consulta acerca desta patologia. A asma grave revela-se um tema actual e deveras pertinente, dada a necessidade em conhecê-la melhor, quer em termos biológicos, clínicos ou fisiopatológicos, o que nos permitiria defini-la e categorizá-la melhor. Uma compreensão aprofundada acerca da asma grave possibilitaria uma actuação mais eficaz, bem como o desenvolvimento de novas alternativas terapêuticas, contribuindo para um melhor controlo da doença. É de salientar as limitações que a asma grave impõe aos que dela padecem em termos de qualidade de vida, assim como o impacto económico que acarreta, apesar de corresponder apenas a uma minoria dos doentes asmáticos. Ainda neste trabalho pretende-se fazer uma breve referência à resposta ao omalizumab instituída a um grupo seleccionado de doze doentes com asma grave do Hospital de Dia do Serviço de Imunoalergologia dos Hospitais de Universidade de Coimbra.

This work aims at providing full and reliable information about the severe asthma phenotypes and their new approach therapies, with particular interest in the emerging biological therapy with omalizumab, as well as a brief reference to asthma epidemiology and diagnosis of difficult to control asthma. This work was based on an updated review of the literature in order to create a new literary source query about this disease. Severe asthma appears to be a topical and very relevant issue, given the need to know it better, both in terms of biological, clinical and pathophysiological features, enabling us to define it and categorize it better. A thorough understanding about the severe asthma would allow greater medical acting, as well as the development of new therapies to better control the disease. It should be noted the limitations severe asthma imposes on those who suffer from it, in terms of quality of life and the economic impact it causes, although it only surprisingly corresponds to a minority of asthma patients. In this study we intend also to make a brief reference to the clinical response to omalizumab in a selected group of twelve patients with severe asthma treated in the Day Hospital Service of Allergology of the University Hospitals of Coimbra, Portugal.
