Sinais ungueais de doenças sistémicas

Abstract

As unhas são estruturas epiteliais cutâneas especializadas e têm como funções a protecção da extremidade dos dedos e a manipulação fina. São facilmente inspeccionadas num exame objectivo, podendo dar diversas informações ao médico. Alterações na cor, forma ou textura das unhas podem ser sugestivas de uma grande variedade de doenças sistémicas. É, por isso, útil saber identificar essas alterações para que seja possível fazer um diagnóstico célere e acompanhamento adequado do doente. São exemplos os dedos em baqueta de tambor, que muitas vezes sugerem doença pulmonar ou doença intestinal inflamatória crónica, e a coiloníquia classicamente associada a carência marcial. O principal objectivo deste trabalho consiste na elaboração de uma revisão das manifestações ungueais que fornecem pistas para o diagnóstico de doenças sistémicas

Nails are specialized keratinous appendages, whose functions are the protection of the tip of the digits and the manipulation of smaller objects with great precision. They are easily inspected in a physical examination, providing a great deal of information that can be useful to the physician. Changes in colour, shape or texture of the nails can suggest a wide range of systemic diseases. Therefore, it is useful to identify those changes so that it is possible to make an early diagnosis, as well as an appropriate monitoring of the patient. A good example is clubbing, a sign that, in most of the cases, suggests pulmonary disease or inflammatory bowel disease. Another classic example is koilonychias, commonly associated to iron deficiency. The main goal of this paper is to elaborate a revision of the nail manifestations that supply clues to the diagnosis of systemic diseases
