Refluxo gastroesofágico e doença respiratória

Abstract

A doença do refluxo gastroesofágico é comum e apresenta-se frequentemente com sintomas típicos de pirose e regurgitação; no entanto, alguns doentes têm sintomas extraesofágicos que se manifestam através de doenças respiratórias. A fisiopatologia da relação entre refluxo e doença respiratória é complexa, o diagnóstico é um desafio e a terapêutica difícil, o que levou a uma extensa pesquisa procurando esclarecer estas vertentes. O objetivo deste trabalho visou assim reunir a informação mais pertinente dessa pesquisa, tendo sido realizada uma revisão de algumas das doenças respiratórias mais frequentemente associadas à doença do refluxo gastroesofágico, nomeadamente a asma, a fibrose pulmonar idiopática, a doença pulmonar obstrutiva crónica, a transplantação pulmonar e a tosse crónica. Sumarizaram-se os aspetos mais relevantes na clínica, fisiopatologia, diagnóstico e tratamento destas entidades, usando a evidência científica mais recente. Gastroesophageal reflux disease is common and often presents with typical symptoms of heartburn and regurgitation; however, some patients have extraesophageal symptoms which become clear through respiratory disorders. The pathophysiology of reflux and its relationship with respiratory diseases is complex, the diagnosis challenging and therapeutics difficult. Because of that, there has been an extensive research trying to clarify these mechanisms. The main goal of this work was to gather the best information concerning that research, through the appraisal of some of the respiratory diseases more frequently associated with gastroesophageal reflux disease, such as asthma, idiopathic pulmonary fibrosis, chronic obstructive pulmonary disease, lung transplantation and chronic cough. The most relevant aspects in clinics, pathophysiology, diagnosis and treatment concerning these entities were summarized using the most recent scientific evidence.
