In addition to directly affecting the inner ear, the disease can arise due to the deposition of immune complexes, as well as being an integral part of other immune-mediated systemic diseases affecting the inner ear.At a clinical level, the patient, in addition to deafness, may present with vestibular symptoms or symptoms characteristic of other autoimmune diseases. The diagnosis, being a diagnosis of exclusion, is essentially based on the clinic and the response to corticosteroid therapy, the start of which will have to be as early as possible to become effective and eventually reverse the progression of the disease. It should be noted that the audiogram, laboratory tests and imaging tests, such as MRI, can also help, but they still require investigation to be endowed, on their own, with diagnostic value. Other therapies have been studied, such as cytotoxic drugs, biological agents, plasmapheresis, cochlear implants, so far without satisfactory results.Therefore, this work aims to carry out a comprehensive review and clarify the aspects of the disease that deserve clinical investigation, particularly with regard to pathophysiological mechanisms, diagnostic and therapeutic approaches, as well as the main future perspectives. A Doença Autoimune do Ouvido Interno (DAIOI) define-se como uma patologia responsável por surdez neurossensorial bilateral, assimétrica e progressiva (no espaço de semanas a meses), na qual estão envolvidos vários mecanismos imunológicos, como respostas humorais, celulares, expressão de citocinas e moléculas de adesão intersticial, ainda pouco estudados e estabelecidos. A doença pode surgir por deposição de complexos imunes, bem como, fazer parte integrante de outras doenças sistémicas imunomediadas com atingimento do ouvido interno. A nível clínico, o doente, para além da surdez, pode ter sintomas vestibulares ou sintomas característicos de outras doenças autoimunes. O diagnóstico, sendo um diagnóstico de exclusão, baseia-se essencialmente na clínica e na resposta à corticoterapia, cujo início terá que ser o mais precocemente possível para se tornar eficaz e poder eventualmente reverter a evolução da doença. De referir que o audiograma tonal simples, os testes laboratoriais e os exames imagiológicos, como a RM podem auxiliar o diagnóstico, mas carecem ainda de investigação para serem dotados, por si só, de valor diagnóstico. Outras terapêuticas foram estudadas, como fármacos citotóxicos, agentes biológicos, plasmaferese, implante coclear, até agora sem resultados satisfatórios. Deste modo, este trabalho tem por objetivo a realização de uma revisão compreensiva e o esclarecimento das vertentes da doença a merecer investigação clínica, nomeadamente no que diz respeito aos mecanismos fisiopatológicos, abordagem diagnóstica e terapêutica, assim como as principais perspetivas futuras. |
