A Direção Executiva do Serviço Nacional de Saúde - Uma primeira análise exploratória tendo por base uma perspetiva de governação

Abstract

Health systems continually face diverse challenges and need more active reforms and policies. The complexity of these adversities requires a better commitment to health governance and requires good health governance, both nationally and globally. Given the importance of health governance, it is essential that there is an evaluation of this always taking into account that each nation has its own health system. This makes sense to have a continuous improvement of health, identify the problems or even record the successes achieved. The biggest challenge is to find a common "language" that is perceptible to all stakeholders who evaluate health work, so we think that the Governance Analytical Framework (GAF) is the ideal methodology to investigate whether the new governance process will be ideal for our health system.Since 1976, the State has had the right to health protection, particularly since the creation of the National Health Service (NHS). Almost 30 years have passed since the publication of the last Statute and the health sector has undergone some transformations, so there was a need for some changes in this area. With the new Statute of the NHS, the Executive Direction of the NHS is considered one of the main innovations with the purpose of working on the operational coordination of the responses of the NHS. This is composed of five departments and directed by the Executive Director Dr. Fernando Araújo. The function of the Executive Direction brings a set of changes to the governance model of the NHS that involve transformations in various areas and functions of other organisms.In this dissertation, we propose a conceptual and empirical approach that makes it possible to study and investigate this innovation in the Portuguese National Health Service and also to understand, from a health governance perspective, the potential impacts of this measure on the NHS. We also seek to apply the practical methodology of the Governance Analytical Framework (GAF) to the Executive Direction. Being an innovation, information about this theme is scarce, for this reason, there was a selection of the media on the subject and, subsequently, we applied some tools of this methodology to the data collected.

Os sistemas de saúde enfrentam continuamente diversos desafios e precisam de reformas e políticas mais ativas. A complexidade destas adversidades obriga a um melhor empenho da governação em saúde e exige uma boa governação em saúde., tanto a nível nacional como a nível mundial. Dada a importância da governação em saúde, é fundamental que haja uma avaliação desta tendo sempre em consideração que cada nação tem o seu sistema de saúde. Esta faz sentido para haver uma melhoria continua da saúde, identificar os problemas ou até registar os sucessos alcançados. O maior desafio está em encontrar uma “linguagem” comum percetível a todos os intervenientes que avaliam o trabalho na saúde, por isso, achamos que o Quadro Analítico de Governação (QAG) seja a metodologia ideal para investigar se o novo processo de governação será o ideal para o nosso sistema de saúde.Desde 1976 que incumbe ao Estado o direito à proteção da saúde, nomeadamente desde a criação do Serviço Nacional de Saúde. Passaram quase 30 anos desde a publicação do último Estatuto e o setor da saúde português tem atravessado por algumas transformações, por isso, houve a necessidade de algumas mudanças nesta área. Neste sentido, com o novo Estatuto do SNS, a Direção Executiva do SNS (DE-SNS) é considerada uma das principais inovações com o propósito de trabalhar na coordenação operacional das respostas do SNS. Esta é composta por cinco órgãos e dirigida pelo Diretor Executivo Dr. Fernando Araújo. A função da DE-SNS traz consigo um conjunto de alterações ao modelo de governo do SNS que envolvem transformações em diversas áreas e funções de outros organismos.Nesta dissertação, propomos uma abordagem conceptual e empírica que possibilite estudar e investigar esta inovação no Serviço Nacional de Saúde português e, ainda, perceber, numa perspetiva de governação em saúde, os potenciais impactos desta medida no SNS. Também procuramos aplicar a metodologia prática do Quadro Analítico de Governação (QAG) à Direção Executiva. Sendo uma inovação, a informação sobre este tema é escassa, por esse motivo, houve uma seleção da comunicação social sobre o tema e, posteriormente, aplicamos algumas ferramentas desta metodologia aos dados recolhidos.
