Abstract

Ao longo das últimas décadas o estudo de terapêuticas para o cancro tem sofrido pronunciáveis avanços, consequência do crescente conhecimento aprofundado das vias e estruturas moleculares responsáveis pela carcinogénese. A identificação dos principais alvos moleculares levou ao desenvolvimento de terapêuticas dirigidas, permitindo um tratamento mais individualizado para cada doente e patologia. Os recetores tirosina cinase têm um papel importantíssimo no crescimento e diferenciação celular. No entanto, podem ser um alicerce no desenvolvimento de doenças neoplásicas. A terapêutica dirigida com inibidores da tirosina cinase torna-se, por isso, o novo paradigma do tratamento do doente oncológico. Ao longo desta revisão, destacarei os principais alvos tirosina cinase, revelando a importância da caracterização genética e molecular do doente e da patologia. Para além disso, irei elucidar os sistemas mais afetados pela toxicidade desta classe farmacológica. A gestão e monitorização da toxicidade sentida é um ponto chave para uma terapêutica efetiva e segura e para tal o farmacêutico tem uma intervenção imprescindível. Este tem um papel fulcral na promoção do bem-estar do doente, através da educação e adesão à terapêutica, gestão dos efeitos adversos e estabelecimento de medidas farmacológicas e não farmacológicas. É imperativo instruir os profissionais de saúde para a importância do diagnóstico molecular, pois apenas através da caracterização genética do tumor, se conseguem instituir terapêuticas eficazes, seletivas e individualizadas.

Over the last few decades, the study of cancer therapies has suffered significant advances, as a result of the increasing in-depth knowledge of the molecular pathways and structures responsible for carcinogenesis. The identification of the main molecular targets has led to the development of targeted therapies, allowing a more individualized treatment, outlined for each patient and pathology. Tyrosine kinases receptors play an important role in cell growth and differentiation. However, they can be the foundation in the development of neoplastic diseases. Targeted therapy with tyrosine kinase inhibitors has therefore become the new paradigm in the treatment of cancer patients. Throughout this review, I will highlight the main tyrosine kinase targets, revealing the importance of genetic and molecular characterization of the patient and pathology. Furthermore, I will elucidate the systems most affected by the toxicity of this pharmacological class. The management and monitoring of the toxicity felt is a key point for an effective and safe therapy and for that the pharmacist has an essential intervention. It has an important role in promoting the patient's well-being, through education and adherence to therapy, management of adverse effects and establishment of pharmacological and non-pharmacological measures. It is imperative to educate health professionals on the importance of molecular diagnosis, because it is only through the genetic characterization of the tumor that effective, selective and individualized therapies can be instituted.
