A Medicina Centrada na Pessoa (MCP) baseia-se no reconhecimento do doente numa perspetiva holística, promove a adesão ao tratamento e melhora as consequências em saúde. O bem-estar dos médicos e a sua saúde mental são fatores importantes no exercício da profissão e na compreensão e valorização do doente e suas doenças. O objetivo deste estudo foi avaliar se a sobrecarga pela gestão da MM estava associada à menos sentida prática da MCP e a sintomas de ansiedade e depressão, o distress, nos médicos de MGF. Métodos: Realizou-se um estudo observacional e transversal numa amostra de conveniência de médicos de MGF através da aplicação on-line de questionários com o fator “Gestão da Consulta” do Questionário de Avaliação da Sobrecarga da Gestão da Multimorbilidade em MGF (SoGeMM-MGF), a dimensão “Compreender a pessoa como um todo” do Questionário de Autoperceção do Desempenho da Medicina Centrada na Pessoa em MGF (MCP) e o Patient Health Questionnaire-4 (PHQ-4), em Agosto de 2021. Para as várias escalas, mais alta pontuação significa melhor estado. Foi realizada estatística descritiva e inferencial. Resultados: Amostra de n=151, 72,2% mulheres (n=109) e 45,0% da amostra até 35 anos (n=68). Verificou-se correlação entre a pontuação total do SoGeMM-MGF e MCP negativa e significativa (ρ=-0,220; p=0,007) e correlação positiva e significativa entre a pontuação total do SoGeMM-MGF e PHQ-4 (ρ=0,320; p<0,001). Verificou-se que a sobrecarga pela gestão da MM é diferente por “Sexo” (feminino:10,47±1,48; masculino:9,55±1,50; p=0,001) e “Número médio de consultas presenciais e/ou remotas por semana a doentes com MM” (<26:9,73±1,87; 26-45:10,09±1,41; >45:10,67±1,46; p=0,026). A MCP teve pontuação significativamente diferente para “Fase de formação” (especialista:7,72±2,11; interno:6,79±1,68; p=0,010) e “Grupo etário” (<36:6,82±1,73; 36-50:7,80±2,13; >50:8,50±2,11; p<0,001). Discussão: Os presentes resultados, pioneiros, devem ser cuidadosamente lidos e apontam para a realização de mais estudos que permitam investigar estratégias para a redução da sobrecarga pela gestão dos doentes com MM, evitando o impacto negativo no exercício da MCP e na saúde mental dos médicos de MGF, até por não haver de momento estudos para comparação de resultados. Conclusão: Neste estudo a sobrecarga pela gestão da Multimorbilidade associou-se a uma menor autoperceção da realização da MCP e a um maior nível de distress (sintomas de ansiedade e depressão) nos médicos de MGF. Introduction: Multimorbidity (MM) is becoming ever prevalent, its management being complex and demanding and associated particularly in the context of General Practice/Family Medicine (GP/FM). Patient-centered Medicine (PCM) is about recognizing the patient in a holistic perspective, promotes adherence to treatment and improves health outcomes. The well-being of physicians and their sound mental health are important factors in patient’s provided care, particularly in understanding the patient and valuing their illness. The aim of this study was to assess if the burden of the management of MM was associated with a less felt practice of PCM and to symptoms of anxiety and depression, the distress, amongst GP/FM physicians. Methods: An observational, cross-sectional study was conducted in a convenience sample of GP/FM physicians through the application on-line in dedicated professional sites of a questionnaire with the factor “Consultation management” of the Questionnaire of Evaluation of Burden of Management of Multimorbidity in General and Family Medicine (SoGeMM-MGF), the dimension “Understanding the whole person” of the Questionnaire of Self-Awareness of Performing Patient-Centered Medicine in General Practice / Family Medicine (PCM), and the Patient Health Questionnaire-4 (PHQ-4), in August of 2021. For all questionnaires higher score means better result. Data was analysed using descriptive and inferential statistics. Results: Sample of n=151, 72,2% women (n=109) and 45,0% up to 35 years (n=68). We found a negative and significant correlation between the total score of SoGeMM-MGF and PCM (ρ=-0,220; p=0,007) and a positive and significant correlation between the total score of SoGeMM-MGF and PHQ-4 (ρ=0,320; p<0,001). The burden of management of MM is different according to “Sex” (female:10,47±1,48; male:9,55±1,50; p=0,001) and “Average number of consultations in person/remote per week with patients with MM” (<26:9,73±1,87; 26-45:10,09±1,41; >45:10,67±1,46; p=0,026). PCM scored significantly different between “Being an attending physician or a resident” (attending physician:7,72±2,11; resident:6,79±1,68; p=0,010) and “Age group” (<36:6,82±1,73; 36-50:7,80±2,13; >50:8,50±2,11; p<0,001). Discussion: The present results, pioneering, must be carefully interpreted and point out the need for further studies that investigate strategies for reducing the burden of management of patients with MM, avoiding the negative impact on the practice of PCM and on GP/FM physicians’ mental health. Conclusion: In this study the burden of management of Multimorbidity was associated with a lower self-awareness of performing PCM and a higher level of distress (symptoms of anxiety and depression) of GP/FM physicians. |
