Acompanhamento do Doente em Consulta na Unidade de uma Equipa Intra Hospitalar de Suporte em Cuidados Paliativos - Monitorização da Sua Evolução e Visão da Família

Abstract

Introdução: Os Cuidados Paliativos (CP) são uma resposta que tem assumido protagonismo com o aumento da longevidade. O crescimento de doenças crónicas, aliado ao aumento das neoplasias, leva à urgente mudança de paradigma e à necessidade de integrar os CP logo no início do curso da doença.Objetivos: Face à problemática descrita, surge o presente estudo que tem como objetivos principais: verificar a importância da referenciação precoce no acompanhamento dos doentes, na consulta de uma Equipa Intra-Hospitalar de Suporte em Cuidados Paliativos (EIHSCP); investigar qual a visão da família sobre a importância do acompanhamento do seu familiar na consulta de EIHSCP. O projeto de investigação propõe-se ainda a dar resposta às seguintes questões: qual a eficácia/ganhos do doente seguido em consulta na unidade da EIHSCP? Qual a perspetiva da família relativamente ao seguimento do doente na consulta da EIHSCP?Metodologia: Para responder às questões levantadas delineou-se um estudo observacional, analítico, transversal, de carácter misto onde a análise dos instrumentos IPOS e EORTC QLQ-C15-PAL, aplicados ao doente é quantitativa e a análise das entrevistas realizadas à família é qualitativa. O estudo decorreu na unidade de uma EIHSCP entre maio e agosto de 2021. Resultados: Foram aplicados os instrumentos de medição a 33 doentes, concluindo o estudo 14 (43%) doentes da amostra. As doenças oncológicas predominaram, sendo a mais comum a neoplasia do intestino, seguindo-se as do sistema digestivo, pâncreas e os colangiocarcinomas. O familiar de referência (FR) era em 85% dos casos do sexo feminino, tendo sido realizada e entrevista a 6 FR. Discussão e Conclusões: Os doentes acompanhados em consulta de CP apresentaram uma evolução positiva nos domínios do controlo sintomático, cognitivo e espiritual. Relativamente à visão da família, verificou-se que a consulta é vista como uma importante fonte de apoio de resposta às necessidades do doente e família, com impacto positivo na melhoria da qualidade de vida do doente.

Introduction: Palliative care (PC) is a response that assumes the important with the increase in longevity. The growth of chronic diseases allied to the increase in neoplasms, leads to necessary to change the paradigm and integrate PCs in the beginning of the disease. Objectives: Faced with this problem, the present study arises, whose main objectives are: to verify the importance of early referral in the follow-up of patients in the consultation of an Intrahospital Support Team (EIHSCP). Also, and to investigate the family`s view of the importance monitoring your family member in the EIHSCP consultation. The research project also proposes to answer the following questions: what is the effectiveness/gains of the patient followed in consultation at the EIHSCP unit? What is the family´s perspective regarding the patient´s follow-up in the EIHSCP consultation? Methodology: To answer the questions raised, an observational, analytical, cross-sectional, mixed study was designed. The analysis of the IPOS and EORTC QLQ-C15-PAL instruments applied to the patient it`s quantitative and the analysis of the interviews carried out with the family it´s qualitative. The study took place at an EIHSCP unit between May and August 2021.Results: The measuring instruments were applied to 33 patients, however only 14 (43%) patients in the sample completed the study. Oncological diseases predominated, the most common being bowel cancer, followed by those of the digestive system, pancreas and cholangiocarcinoma’s. In 85% of the cases, the relative of reference (FR) was female, and 6 FR were interviewed.Discussion and Conclusions: It was concluded that patients followed um in a PC consultation showed a positive evolution in the domains of symptomatic, cognitive and spiritual control. Regarding the view of the family, it was found that the consultation is seen as an important source of support in response to the patient´s and family’s needs, evidenced by the family as an improvement in the patient`s quality of life.
