dc.description.abstract | A síndrome de transfusão feto-fetal é uma complicação rara, que afeta 10-15% das gestações gemelares com placenta monocoriónica. Esta patologia desenvolve-se quando através das anastomoses presentes na placenta, nomeadamente as arterio-venosas, existe fluxo sanguíneo desequilibrado de um feto para o outro. Assim, o feto que recebe o fluido sanguíneo (recetor) torna-se hipervolémico enquanto o seu co-gémeo (dador) se torna hipovolémico. Estas alterações hemodinâmicas vão gerar mecanismos fisiopatológicos compensatórios em ambos os fetos. O diagnóstico é feito através da avaliação ecográfica, sendo o principal critério a discrepância do volume de líquido amniótico entre as duas cavidades: o dador apresenta uma bolsa vertical máxima inferior a 2 cm, enquanto a bolsa vertical máxima do recetor é superior a 8 cm. A ecografia permite também a classificação em 5 estádios, ordenados por ordem crescente de gravidade. Quando não tratada apresenta alto risco de mortalidade e morbilidade perinatal. Contudo, com a fotocoagulação a laser das anastomoses placentárias é possível atingir taxas de sobrevida global entre os 50%-70%. Existe evidência de que este é o tratamento que permite melhor sobrevida fetal e menores complicações pós-natais, especialmente nos estádios II-IV. Porém, no estádio I a maioria dos centros opta por uma atitude expectante, dado que ainda não existe consenso sobre a forma de atuar perante este estádio, sendo necessários mais estudos para definir se a fotocoagulação nestes casos terá mais vantagens que a atitude expectante com vigilância. | por |
dc.description.abstract | Feto-fetal transfusion syndrome is a rare complication, affecting 10-15% of monochorionic twin pregnancies. This pathology develops when, through the anastomoses present in the placenta, especially arterio-venous, there is an unbalanced blood flow from one fetus to another. Thus, the fetus that receives fluid blood (recipient) becomes hypervolemic while its co-twin (donor) becomes hypovolemic. These hemodynamic changes will generate compensatory pathophysiological mechanisms in both fetuses. The diagnosis is made through ultrasound evaluation; the main criterion is the discrepancy in the volume of amniotic fluid between the two cavities: the donor has a maximum vertical pocket inferior to 2 cm, while the maximum vertical pocket of the recipient is greater than 8 cm. Ultrasound evaluation also allows classification into 5 stages, arranged in ascending order of severity. When untreated, it presents a high risk of perinatal mortality and morbidity. However, with laser photocoagulation of placental anastomoses, it is possible to achieve overall survival rates between 50% -70%. There is evidence that this is the treatment that allows better fetal survival and less postnatal complications, particularly in stages II-IV. However, in stage I, most centers opt for an expectant attitude, since there is still no consensus on how to act in this stage, and further studies are needed to define whether a photocoagulation will have more advantages than the expectant attitude with surveillance. | eng |
