Abstract: Introdução: A vasculopatia polipóide da coroideia (VPC) é uma patologia corioretiniana que se define pela presença de uma rede neovascular anómala associada a dilatações aneurismáticas, que se desenvolve entre a coriocapilar e o epitélio pigmentar da retina (EPR), e que tradicionalmente se associa a descolamentos sero-hemorrágicos no polo posterior. Alterações coroideias concomitantes, uma vez presentes, podem também afetar o olho adelfo, apesar de pouco descritas, principalmente em populações caucasianas. Assim, este trabalho tem como objetivo realizar, numa amostra de doentes caucasianos com VPC unilateral, a caracterização imagiológica multimodal dos olhos adelfos não afetados.Métodos: Estudo de coorte retrospetivo multicêntrico com uma amostra de 55 olhos adelfos não afetados de doentes com diagnóstico de VPC unilateral confirmado por Angiografia com Verde de Indocianina (AVI). A amostra foi caracterizada na baseline por retinografia, tomografia de coerência ótica de domínio espectral (SD-OCT), angiografia fluoresceínica e com verde de indocianina. Foram avaliadas características morfológicas tanto da retina como da coróide em âmbito multimodal. Foi ainda avaliado o SD-OCT da última visita de seguimento destes doentes de forma a excluir evolução para VPC/neovascularização coroideia. Resultados: Da nossa amostra inicial (77 doentes), 9 (11,7%) dos doentes apresentavam VPC bilateral. Os 55 doentes incluídos apresentavam uma idade mediana de 7415 anos. Após 15,56,4 meses de seguimento, apenas um desenvolveu doença no olho adelfo (1,9%). Drusen moles estavam presentes em 32,7% dos olhos adelfos, drusen duros em 27,3%, pseudodrusen reticulares em 5,4% e paquidrusen em 23,6%. Alterações pigmentares estavam também presentes em 29,1% dos casos. A espessura da retina central foi de 273,037,0m, a da coróide subfoveal foi de 238,983,0m, estando presentes sinais de paquicoróide (tanto focal como difusa) em 47,2% da amostra. Identificámos o sinal da dupla camada em 36,4% dos casos, elevação por descolamento do epitélio pigmentado da retina em 5,4%, disrupção do EPR por epiteliopatia/alterações pigmentares em 16,4% e presença de fluido em 9,1% dos doentes. A atrofia do EPR estava presente em 10,9% da amostra. Na AVI, a dilatação vascular da coróide estava presente em 63,6% dos pacientes, a hiperfluorescência difusa em 49,1%, a hiperfluorescência puntiforme em 52,7%, anastomoses no polo posterior ocorreram em 18,2% dos casos e hiperfluorescência geográfica tardia em 16,4%. Rede vascular anómala foi identificada em apenas 1,8% dos casos. Conclusões: Os sinais de paquicoróide no OCT e na AVI estiveram presentes em mais de metade dos casos e o sinal da dupla camada em mais de um terço. Constituem achados muito relevantes para a melhor caracterização desta patologia e para a compreensão da sua patogénese, podendo traduzir diferentes fases da sua progressão.
Introduction: Polypoid choroidal vasculopathy (VPC) is a chorioretinal pathology that is defined by the presence of an anomalous neovascular network associated with aneurysmal dilations, which develops between the choriocapillary and the retinal pigment epithelium (EPR), and which has traditionally been associated with sero-hemorrhagic detachments at the posterior pole. Concomitant choroidal changes, once present, can also affect the fellow eye, lacking characterization especially in Caucasian populations. Thus, this work aims to perform, in a sample of Caucasian patients with unilateral PCV, the multimodal imaging characterization of unaffected fellow eyes.Methods: Multicenter retrospective cohort study with a sample of 55 unaffected fellow eyes from patients diagnosed with unilateral PCV confirmed by Indocyanine Green Angiography (AVI). The sample was characterized in the baseline by color fundus photography, spectral domain optical coherence tomography (SD-OCT), fluorescein angiography and with indocyanine green. Morphological characteristics of both the retina and the choroid were evaluated in multimodal scope. The SD-OCT of the last follow-up visit was also evaluated in order to exclude evolution to VPC / choroidal neovascularization.Results: From our initial sample (77 patients), 11.7% of patients had bilateral VPC. The 55 patients included had a median age of 7415 years. After 15.56.4 months of follow-up, only one developed disease (1.9%). Soft drusen were present in 32.7% of the fellow eyes, hard drusen in 27.3%, reticular pseudodrusen in 5.4% and pachydrusen in 23.6%. Pigmentary changes were also present in 29.1% of the cases. The thickness of the central retina was 273.037.0m, subfoveal choroid thickness was 238.83.0m, with signs of pachychoroid (both focal and diffuse) present in 47.2% of the sample. We identified the double layer sign in 36.4% of the cases, elevation due to detachment of the retinal pigmented epithelium in 5.4%, disruption of the EPR due to epitheliopathy / pigmentary changes in 16.4% and presence of fluid in 9.1% of the cases. EPR atrophy was present in 10.9% of the sample. In AVI, choroidal vascular dilation was present in 63.6% of patients, diffuse hyperfluorescence in 49.1%, punctiform hyperfluorescence in 52.7%, anastomoses in the posterior pole occurred in 18.2% of cases and late geographic hyperfluorescence in 16.4%. Branching vascular network was identified in only 1.8% of cases.Conclusions: The pachychoroid signs in the OCT and AVI were present in more than half of the cases and the double layer sign in more than a third. They are very relevant findings for the better characterization of this pathology and for the understanding of its pathogenesis, being able to translate different phases of its progression.
