Abstract: A síndrome do olho seco é uma doença crónica e autoimune que afeta a glândula lacrimal, causando uma diminuição na produção do filme lacrimal, e que atinge entre 5 a 50% da população mundial.O filme lacrimal é uma camada de fluído fina constituída por 98% de água e 2% de proteínas e lípidos. Num indivíduo que sofre da síndrome de olho seco prevê-se que o volume de filme lacrimal seja inferior ao produzido num indivíduo sem esta limitação e, portanto, estima-se que a taxa de filme lacrimal para olho seco é de 0,27 mL/dia, e para olho normal de 2,8 mL/dia. A síndrome do olho seco não tem cura, contudo já existem formas de atenuar os sintomas. Os tratamentos consistem na utilização lágrimas artificiais ou colírios. Porém, quando a patologia é mais grave o paciente pode recorrer a fármacos de administração oral ou tópica, a cirurgias oculares ou à inserção de um dispositivo ocular. Os principais compostos presentes nos métodos de tratamento do olho seco são: a Hidroxipropilmetilcelulose (HPMC), o Ácido Hialurónico (HA), o Propilenoglicol (PG), o Glicerol, o Polietilenoglicol (PEG) e a Policaprolactona (PCL).Os insertos oculares são considerados potenciais sistemas de tratamento de doenças oculares. Estes são formulações sólidas ou semi-sólidas, de estrutura fina, com mono ou multicamadas e de pequenas dimensões e constituídos por um suporte polimérico, que pode ser do tipo insolúvel, solúvel ou bioerodível.A presente Dissertação encontra-se inserida no âmbito do projeto Ineye®, desenvolvido no Departamento de Engenharia Química da Faculdade de Ciências e Tecnologia da Universidade de Coimbra.O objetivo deste estudo incide na formulação de uma metodologia da composição de um inserto oftálmico que, aquando da sua colocação na pálpebra inferior do olho, dê início ao processo de libertação, de forma controlada, dos lubrificantes contidos no seu interior. Pretende-se que essa libertação seja feita a uma taxa constante, durante pelo menos 15 dias, de forma a atenuar os sintomas característicos da síndrome do olho seco. O inserto deve ser do tipo insolúvel, biocompatível, não tóxico, não alergénico, e não deve provocar a sensação de “areia no olho”.Para o efeito foram testados vários tipos de insertos: insertos compostos por um núcleo constituído por substâncias ativas (lubrificantes oculares) revestidos por um polímero que pode ser só de PCL ou de PCL/PEG em diferentes proporções de PCL e de PEG; e insertos de uma mistura de lubrificantes e PCL, também em diferentes proporções. As misturas de PCL e PEG são realizadas fisicamente ou sintetizadas quimicamente pelo mecanismo da reação de abertura de anel por coordenação-inserção. Os métodos de caracterização aplicados neste trabalho são: a Determinação da Energia de Superfície através da medição de ângulos de contacto; a Ressonância Magnética Nuclear (RMN) e avalia-se a Perda de massa do inserto e a Capacidade de absorção de água, utilizando um meio de dissolução que mimetiza as lágrimas.A partir dos ensaios realizados constatou-se que os polímeros de revestimento com elevada quantidade de PEG não se adequam ao que se pretende, e, de entre os vários pesos moleculares de PEG, o que possui 6000 g/mol é o mais apropriado. A formulação composta por uma mistura física 80%PCL/20%lubrificantes mostrou ser a mais acertada para o que se pretende. Como trabalhos futuros será pertinente incluir a Carboximetilcelulose (CMC) e o Dextrano na lista de substâncias ativas.
Dry eye syndrome is a chronic and autoimmune disease that affects the lacrimal gland, causing a decrease in the production of the tear film, and affecting about 5 to 50% of the world´s population.The tear film is a thin fluid layer made up of 98% of water and 2% of proteins and lipids. In an individual who is suffering from dry eye syndrome, the volume of the tear film is expected to be less than the produced in an individual without this condition, and therefore it is estimated that the tear film rate of dry eye is 0,27 mL/day and for a normal eye is 2,8 mL/day.Dry eye syndrome has no cure, however there are many ways to relieve symptoms. The treatments consist of using artificial tears or eye drops, however, when the pathology is more severe, the patient may resort to oral or topical drugs, eye surgery or the insertion of an eye device.The main compounds present in the dry eye treatment methods are: Hydroxypropylmethylcellulose (HPMC), Hyaluronic Acid (HA), Propylene Glycol (PG), Glyceril, Polyethylene Glycol (PEG) and Polycaprolactone (PCL).Eye inserts are considered to be potential eye disease treatment systems. These are solid or semi-solid formulations, of thin structure, with mono or multilayer and small dimensions and consisting of a polymeric support, which can be insoluble, soluble or bioerodible.This dissertation is part of the Ineye® project, developed in the Chemical Engineering Department of the Faculty of Sciences and Technologie of the University of Coimbra.The objective of this study is to formulate a methodology for the composition of an ophthalmic insert, which when placed on the lower eyelid of the eye, initiates the process of releasing, in a controlled manner, the lubricants contained inside. And it is intended that this release is done at a constant rate, for at least 15 days, in order to mitigate the symptoms of dry eye syndrome. The insert must be of the insoluble, biocompatible, non-toxic, non-allergenic type and must not cause the sensation of “sand in the eye”. For this purpose, several types of inserts were tested: inserts composed of a core consisting of active substances (ocular lubricants) coated with a polymer that can be either PCL or PCL / PEG in different proportions of PCL and PEG; and inserts from a mixture of lubricants and PCL, also in different proportions. The mixtures of PCL and PEG are carried out physically or chemically synthesized by the mechanism of the ring-opening reaction by coordination-insertion.The characterization methods applied in this work are: the Determination of the Surface Energy through the measurement of contact angles; the Nuclear Magnetic Resonance (NMR) and the Process of mass loss of the insert and the Water absorption capacity are evaluated, using a dissolution formulation that mimics tears. From the tests carried out it was found that coating polymers with a high amount of PEG are not suitable for the purpose what is intended and among the various molecular weights of PEG, the one that has 6000 g/mol is the most appropriate. The formulation composed of a physical mixture of 80%PCL/20%lubricants proved to be the most accurate for what is intended. As future work it will be pertinent to include Carboxymethylcellulose (CMC) and Dextran in the list of active substances.
