dc.description.abstract | Introdução: A polimedicação define-se como a toma diária de 5 ou mais medicamentos, estando este número a aumentar com o envelhecimento da população. Isto acarreta riscos, nomeadamente aumento das interações farmacológicas. Surge, assim, o conceito de desprescrição que visa reduzir ou descontinuar uma medicação considerada desnecessária ou de risco.Objetivos: Avaliar a vontade dos idosos em ter medicação desprescrita e perceber quais as atitudes do médico de família que na perspetiva do idoso irão influenciar o processo de desprescrição. Materiais e Métodos: Foi realizado um estudo observacional descritivo e transversal, com a recolha de dados feita com recurso a um questionário em papel, construído a partir da literatura revista. Foram aplicados em 3 USF, 1 pertencente à Unidade de Saúde da Ilha Terceira e 2 pertencentes ao Agrupamento de Centros de Saúde do Baixo Mondego. A população foi constituída por doentes com idade ≥ 65 anos e com toma de ≥ 5 medicamentos de forma crónica. Os dados foram codificados e analisados através do software estatístico SPSS® versão 25, tendo sido aplicada estatística descritiva e indiferencial (teste de Mann-Whitney, Kruskal-Wallis, Qui-Quadrado e o coeficiente de correlação de Pearson).Resultados: Foram recolhidos 87 questionários. A idade média foi de 76.6±6.8 anos, sendo 66.7% do sexo feminino. Para 27.6% dos participantes havia conhecimento do que é desprescrição. Cerca de 54.0% (vs 35.6%) afirma que os medicamentos fazem mais bem do que mal; 9.2% estão interessados em ter medicação desprescrita, enquanto que 46.0% são contra este processo. Saber o que é desprescrição e a vontade em ter medicação desprescrita apresentaram uma associação com a educação formal [maior formação] (p<0.001 e p=0.021 respetivamente) e com a área de residência [Angra] (p=0.007 e p=0.013, respetivamente). Discussão e Conclusão: Verificou-se que os idosos apresentavam pouca vontade em ter medicação desprescrita e que as atitudes do médico de família que influenciavam a desprescrição na perspetiva do idoso são: “sei o que é desprescrição” e “tenho uma boa relação com o médico”. É necessário sensibilizar e educar tanto os médicos (através da divulgação dos algoritmos de desprescrição já existentes) como a população (através da explicação do termo desprescrição). Foi percetível que o MF refere sobretudo os benefícios da medicação. Contudo, é obrigação do MF a capacitação das pessoas, ao alertar para os riscos da medicação, para que possa haver uma decisão terapêutica partilhada. | por |
dc.description.abstract | Introduction: Polymedication is defined as the daily intake of 5 or more drugs, and this number is rising as the population ages. This comes with risks, namely the increase of pharmacological interactions. Thus the concept of un-prescription, which aims to lower or suspend a drug considered unnecessary or risky.Objectives: To evaluate the will of the elderly to have their medication un-prescribed and understand, in their perspective, the actions of their family doctor which influence the process of un-prescription.Materials and Methods: An observational, descriptive and cross-sectional study was designed, and data collection used a paper questionnaire, built from reviewed literature. It was applied to 3 Family Health Units, 1 belonging to the Health Unit of Terceira Island and 2 belonging to the group of Healthcare Centers (ACeS) of the Lower Mondego. The population was composed of patients aging ≥ 65 years and with ≥ 5 chronic drugs. Data was coded and analyzed through the statistical software SPSS® version 25, and descriptive and non-differential statistic was applied (Mann-Whitney, Kruskal-Wallis, ~Chi-Square tests and Pearson’s correlation coefficient).Results: 87 questionnaires were gathered. The average age was 76.6±6.8 years, 66.7% of female gender. 27.6% of the participants were aware of un-prescription. About 54.0% (vs 35.6%) claim drugs do more harm than good; 9.2% are interested in having their medication unprescribed, while 46.0% are against this process. Knowing what un-prescription is and the desire to have their medication unprescribed showed an association with formal education [higher education] (p<0.001 and p=0.021 respectively) and area of residence [Angra] (p=0.007 and p=0.013, respectively). Discussion and Conclusion: It was noted that the elderly showed little desire to have their medication unprescribed and that the actions of their family doctor (GP) which influenced un-prescription are: “I know what un-prescription is” and “I have a good relationship with my doctor”. It is necessary to sensitize and educate both doctors (through spreading of already existing un-prescription algorithms) and the community (through explication of the concept of un-prescription). It was noticeable that the GP mentions primarily the benefits of medication.
