Abstract: A fibrilhação auricular (FA) é a arritmia cardíaca sustentada mais comum, prevendo-se que, nos países desenvolvidos, 1 em cada 4 adultos de meia-idade venha a desenvolver FA. Em 2030 prevê-se que existam entre 14 e 17 milhões de doentes com FA na UE, com 120 000-215 000 doentes diagnosticados de novo por ano. Sendo a FA uma patologia que se associa ao aumento da mortalidade e morbilidade, nomeadamente ocorrência de AVC, hospitalizações frequentes, disfunção ventricular esquerda, distúrbios cognitivos e insuficiência cardíaca, torna-se essencial a melhoria e optimização do tratamento integrado destes doentes. O tratamento da FA abrange terapêuticas com impacto prognóstico, como sejam a anticoagulação, bem como terapêuticas predominantemente com benefício sintomático, como é o caso do controlo da frequência e o controlo do ritmo cardíacos.Num episódio agudo de FA, com menos de 48 horas de evolução, pode ser considerada cardioversão farmacológica ou eléctrica. No entanto, alguns destes episódios podem corresponder a FA paroxistica em que o episódio arritmico poderia terminar espontaneamente na maioria dos doentes e portanto, em que a cardioversão poderia ser evitada. Atendendo à incerteza em relação ao timing ideal para cardioversão elétrica num episódio agudo de FA, está atualmente em curso o ensaio clínico randomizado ACWAS que pretendeverificar a não inferioridade da estratégia "wait and see" (em que o doente apenas é submetido a controlo da frequência cardíaca, aguardando a conversão espontânea a ritmo sinusal) relativamente à estratégia convencional de cardioversão. Este trabalho final de mestrado integrado em medicina tem como propósito a revisão sistemática da evidência relativamente à cardioversão num episódio de FA aguda, procurando responder à questão: qual o timing ideal para cardioversão elétrica?
Atrial fibrillation (AF) is the most common cardiac arrhythmia and, in developed countries, one in four middle-aged adults are expected to develop this disease. AF is a condition associated with increased mortality and morbidity, including the occurrence of stroke, frequent hospitalizations, cognitive impairment and heart failure, and as such, it becomes essential to improve and optimize the integrated treatment of these patients. In an acute episode of AF, with less than 48 hours of duration, both pharmacological and electrical cardioversion (ECV) can be performed. However, some of these episodes can correspond to paroxysmal AF, a type of AF in which the arrhythmic episode may end spontaneously, as it happens in the vast majority of cases, and therefore, in which cardioversion would not be necessary and could be avoided. Given the uncertainty regarding the ideal timing for ECV in an acute episode of AF, a randomized clinical trial is currently on-going, the ACWAS trial, that intends to verify the non-inferiority of the wait-and-see strategy. This approach is based on submitting patients only to an early rate control strategy, while waiting for spontaneous conversion to sinus rhythm, with the possibility of cardioversion within 48 hours after onset of symptoms, if needed, in comparison to the current standard of care (urgent restoration of sinus rhythm, achieved by pharmacologic or electrical cardioversion). This review aims to provide a systematic appraisal of the evidence regarding cardioversion in an acute episode of AF, so we can try to answer the question: what is the ideal timing for cardioversion?
