Abstract: Purpose: Comparison between the use of two attachments (ball and locator) in molars and premolar IARPD, and consequently study the behavior of the supporting tissues (cortical bone, oral mucosa periodontal ligament) and RPD and respective components (framework, implants, attachments and teflon). The main motivation for this study is the fact that there is not that much information regarding finite element studies of IARPD with different attachments. Method: A three-dimensional model of a molar and pre-molar IARPD was built, with the different attachments. Initially, using CBCT images together with 3D reconstruction and non-parametric processing, it was obtained the three-dimensional mandible model used in this study. The RPD and other components, including attachments, were constructed using Solidworks and assembled on the same software. After assembly, the models were exported to ADINA in the form of binary parasolid files, in order to complete the finite element modeling. Subsequently, bilateral loading of 60 N was simulated on both acrylics of the IARPD. Results: In all results, the null hypothesis, of the values studied between attachments being equal, was rejected. The locator, when used as an attachment, produces a significant reduction of pressure in soft tissue in molar IARPD and also the stress on the implant in a premolar IARPD. Conclusions: The locator attachment reduces the displacement, stresses, pressures and deformations, and may be the attachment that guarantees better long-term results.
Objetivo: Comparação entre o uso de dois attachments (bola e locator) em PPRAI molares e pré-molares, e consequentemente estudar o comportamento dos tecidos de suporte (osso cortical, mucosa oral e ligamento periodontal) e da PPR e respetivos componentes(esqueleto, implantes, attachments, e teflons). A motivação para este estudo foi o facto de haver pouca informação no que diz respeito a estudos com elementos finitos de PPRAI com diferentes attachments. Método: Construiu-se um modelo tridimensional de PPRAI molares e pré-molares, com os diferentes attachments. Inicialmente, com recurso a imagens de CBCT juntamente reconstrução 3D e processamento não-paramétrico, obetve-se o modelo da tridimensional mandíbula utilizada neste estudo. A PPR e restantes componentes, onde estão incluídos os attachments, foram construídos com recurso ao Solidworks e montados nesse mesmo software. Após a montagem, os modelos foram exportados para o ADINA no formato de ficheiros parasólidos binários, para ser efetuada a modelação dos elementos finitos. Posteriormente procedeu-se à simulação de cargas bilaterais de 60N em ambos os acrílicos das PPRAI. Resultados: Em todos os resultados, a hipótese nula, dos valores estudados entre attachments serem iguais, foi rejeitada. O locator quando usado como attachment produz uma redução significativa de pressão no tecido mole numa PPRAI molares e ainda a tensão no implante numa PPRAI pré-molares. Conclusões: O attachment locator reduz o deslocamento, tensões, pressões e deformações, podendo ser o attachment que garante melhores resultados a longo prazo.
