Title: Estratégia diagnóstica e atitude terapêutica face à detecção incidental de massas suprarrenal

Abstract: Contexto: O uso cada vez mais frequente de métodos não invasivos de avaliação morfológica do abdómen, realizados por razões independentes da suspeita de doença na glândula suprarrenal, condicionou um aumento da detecção incidental de massas neste órgão. O seu significado, bem como a estratégia diagnóstica e posterior terapêutica, precisam de ser bem definidos, para facilitar a investigação por parte do profissional de saúde, com uma adequada gestão de recursos e sem causar sofrimento desnecessário ao doente. Objectivo: Os objectivos desta revisão são (1) abordar os diagnósticos mais frequentes perante a detecção incidental de uma massa na suprarrenal, (2) delinear a estratégia para alcançar um diagnóstico definitivo e (3) definir a abordagem terapêutica mais adequada. Métodos: Reviu-se e analisou-se a literatura médica sobre massas da suprarrenal detectadas incidentalmente, abrangendo o período de 2000 a 2012, em língua portuguesa e inglesa, de revistas científicas de diversas áreas e de livros que abrangessem o tema em estudo. Foi utilizada a base de dados PUBMED, o Índex de Revistas Médicas Portuguesas e o Repositório Institucional dos HUC (rihuc.huc.min-saude.pt) com a finalidade de encontrar estudos, artigos de revisão e guidelines acerca do tema em questão. Foram utilizadas as seguintes palavras-chave, em combinações variadas: “aldosteronismo primário” (primary aldosteronism), “carcinoma”, “cirurgia” (surgery) “Cushing”, “feocromocitoma” (pheochromocytoma), “ganglioneuroma”, “guidelines”, “imagiologia” (imaging), “incidental” (incidentally), “incidentaloma”, “massa(s)” (mass/masses), “metástases” (metastasis), “mielolipoma” (myelolipoma), “PET”, “quisto” (cyst) “subclínico” (subclinical), “suprarrenal/suprarrenal” (adrenal), “TC” (CT) e “tratamento” (treatment). Resultados: O fenótipo imagiológico inicial, bem como a história clínica e o exame físico, devem orientar a investigação laboratorial, devendo ser realizada, primordialmente, a exclusão de feocromocitoma, pelos riscos associados a outras técnicas diagnósticas e ao próprio tratamento cirúrgico. A avaliação radiológica com recurso à TC e à RMN fornecem informação útil para a identificação de lesões malignas. Todas as massas funcionantes devem ser removidas cirurgicamente para prevenir morbilidade, bem como massas com uma dimensão superior a 4 cm. A biópsia deve ser utilizada para confirmar ou excluir malignidade nas situações em que os resultados da TC ou da RMN são inconclusivos, após a exclusão bioquímica da existência de feocromocitoma. Conclusão: A investigação cuidada de cada massa da suprarrenal é essencial para evitar potenciais consequências para a saúde dos indivíduos. São necessários mais estudos a longo prazo para avaliar a eficácia dos métodos diagnósticos e das abordagens terapêuticas aplicadas.
Context: The increasingly frequent use of noninvasive methods for morphological evaluation of the abdomen, performed for reasons other than suspected adrenal gland disease, conditioned an increase in the incidental detection of masses in this organ. Its meaning, as well as its diagnosis and subsequent therapeutic strategy, need to be well defined to facilitate the investigation by the health care professional with appropriate management of resources and without causing unnecessary suffering to the patient. Objective: The objectives of this review are (1) to describe the most common diagnoses of an incidentally detected adrenal mass, (2) outline the strategy to reach a definitive diagnosis, and (3) to define the most appropriate therapeutic approach. Methods: We revised up and analyzed the medical literature on the adrenal masses incidentally detected, covering the period from 2000 to 2012, in English and Portuguese, from journals and books of several study areas that covered this topic. The researches were conducted in the PUBMED database, the Index of Portuguese Medical Journals and in the Institutional Repository of HUC (rihuc.huc.min-saude.pt) in order to find studies, review articles and guidelines on the subject in question. We used the following keywords in various combinations: "primary aldosteronism", "carcinoma", "surgery", "Cushing", "pheochromocytoma", "ganglioneuroma", "guidelines", "imaging", "incidental", "incidentaloma," "mass/masses”, "metastasis”, "myelolipoma", "PET", "cyst", "subclinical", "adrenal”, "CT” and “treatment”. Results: The initial imaging phenotype, as well as the clinical history and physical examination, should guide the laboratory investigation, and should be performed primarily to exclude pheochromocytoma, due to the risks associated with other diagnostic techniques and the surgical treatment itself. Radiological evaluation using CT and MRI provide useful information for identifying malignant lesions. All functional masses and those with a size greater than 4 cm must be surgically removed to prevent morbidity. A biopsy should be used to confirm or rule out malignancy in cases where the results of CT or MRI are inconclusive, after biochemical exclusion of the existence of pheochromocytoma. Conclusion: A careful investigation of an adrenal mass is essential to avoid potential health consequences to each individual. Further studies are needed to assess the long term effectiveness of the diagnostic methods and therapeutic approaches
