Title: Avaliação de marcadores bioquímicos de risco inflamatório em mulheres jovens com síndroma dos ovários políquisticos

Abstract: Introdução: A Síndroma dos ovários poliquísticos (SOPQ) é uma patologia endócrina muito comum em mulheres jovens, constituindo não só um problema reprodutivo, mas antes uma patologia complexa e multifacetada. A avaliação de marcadores bioquímicos poderá ser útil para avaliar o risco inflamatório deste grupo de mulheres, com vista a uma compreensão global do risco cardiovascular a que estão sujeitas. Objectivos: Com este estudo pretende-se avaliar a presença de marcadores bioquímicos de risco inflamatório numa população de mulheres jovens com SOPQ, comparando-a com mulheres sem a doença, no decurso da realização de técnicas de Procriação Medicamente Assistida. Pretende-se, assim, verificar se existe ou não uma variação do risco inflamatório no decurso da estimulação ovárica em mulheres jovens portadoras de SOPQ e avaliar o consequente impacto de eventuais alterações bioquímicas detectadas. Metodologia: Numa amostra constituída por 19 mulheres com SOPQ e 88 mulheres sem a doença, seguidas em Consulta de Esterilidade do Serviço de Reprodução Humana dos Hospitais da Universidade de Coimbra e submetidas a estimulação ovárica com gonadotrofinas, procedeu-se à pesquisa de marcadores inflamatórios, nomeadamente, Proteína C-reactiva, fibrinogénio, contagem leucocitária, D-dímeros e plaquetas. A análise estatística foi feita com recurso ao programa Statistical Package for the Social Sciences (SPSS, version 13.0). Resultados: Não se verificou a existência de diferença estatisticamente significativa entre a idade média dos dois grupos de mulheres; o índice de massa corporal médio das mulheres com SOPQ (27,1 kg/m2) é significativamente superior ao do grupo controlo (24,2 kg/m2); o mesmo se constatando relativamente à razão LH/FSH [1,50 (0,80 – 3,29)] e [1,00 (0,72 – 1,60)] respectivamente. A SOPQ não tem influência directa nos valores observados para a PCR, no entanto, ao longo da estimulação ovárica os níveis de PCR aumentam nos dois grupos, sendo este aumento mais acentuado no grupo de mulheres com SOPQ. A SOPQ tem influência directa nos valores observados para o fibrinogénio, sendo que os valores deste parâmetro são mais elevados nas mulheres com SOPQ em qualquer dia do protocolo de estimulação ovárica. A contagem leucocitária aumenta ao longo da estimulação ovárica, sendo esta tendência igual nos dois grupos; as diferenças observadas para a contagem leucocitária nos diferentes dias de colheita são estatisticamente significativas. Os valores de D-dímeros não variam significativamente no decurso do protocolo e entre os dois grupos avaliados. A contagem de plaquetas diminui ao longo do tempo de estimulação ovárica; no entanto, para as mulheres com SOPQ a diminuição é mais acentuada, embora apresentem valores mais elevados que o grupo controlo nos dois primeiros momentos de avaliação. Conclusão: O risco inflamatório de mulheres com SOPQ, está longe de ser uma discussão terminada, pelo que se torna interessante avaliar esta situação aquando da realização de protocolos que, hipoteticamente, possam ainda exacerbar mais este risco, como é o caso da estimulação ovárica com gonadotrofinas. Comparando os dois grupos de mulheres (com e sem SOPQ) conclui-se que as diferenças encontradas nos vários marcadores inflamatórios são inespecíficas uma vez que, para a maioria dos marcadores, a evolução nos dois grupos é sobreponível
Introduction: Polycystic Ovarian Syndrome (PCOS) is a common endocrine disorder that affects predominantly young women and is considered more than a reproductive disorder, a complex and multifaceted disease. Biochemical markers could be useful to analyze the inflammatory risk in this group of women, and allow further insight as well as a global comprehension of the cardiovascular risk to which they are exposed. Purpose: The aim of this study is to analyze the presence of biochemical markers of inflammatory risk in a group of young women with PCOS in comparison to women without this disorder, during medically assisted reproductive techniques. In this way it is possible to examine whether there is a variation in the inflammatory risk during ovarian stimulation in young women with PCOS and therefore allow the analysis of the impact of possible biochemical modifications. Methodology: This study was performed on a sample of 19 women with PCOS and 88 women without this disorder, that were being monitored in the infertility program at the Human Reproductive Unit of the Hospital of the University of Coimbra. This sample of women underwent ovarian stimulation with gonadotropins, and were studied in search of inflammatory markers, more specifically C reactive protein, fibrinogen, leukocyte count, D-dimers and platelets. Statistical analysis was done with the aid of the program “Statistical Package for the social sciences”. Results: The results showed no significant statistical difference between the median ages of the two groups of women. The median for the body mass index of the women with PCOS (27,1 kg/m2) is significantly higher to the control group (24,2 kg/m2) and the same occurred for the LH/FSH ratio, [1,50 (0,80 – 3,29)] and [1,00 (0,72 – 1,60)] respectively. The PCOS does not directly influence the observed values for the C reactive protein, however, during ovarian stimulation, these levels increased in both groups and this increase proved to be more significant in the group of women with PCOS. The variable referring to the presence of PCOS directly influences the observed values registered for the fibrinogen, thus the values of this parameter seem to be higher in the group of women with PCOS on each day of the protocol of ovarian stimulation. Leukocyte count increases with ovarian stimulation showing an equal tendency in both groups, but the differences in leukocyte count on the different days show statistical significance. The D-dimer results do not vary significantly during the protocol and between the two groups. The platelet count decreases with ovarian stimulation time, however for the women with PCOS, there is a greater decline, although they present with higher values in the first two moments of analysis. Conclusion: The inflammatory risk for women with PCOS is a topic thoroughly discussed in the literature, but it is far from a concluding debate which makes it interesting to analyze this situation during the protocols that hypothetically could exacerbate the risk, as is the case of ovarian stimulation with gonadotropins. The comparison of the two groups of women, leads to the conclusion that the differences found in the various inflammatory markers are unspecific since for the majority, the progression in both groups were similar
