Abstract: O estágio curricular é o culminar de um percurso académico de 5 anos. O meu estágio realizou-se na Farmácia Monte Formoso, sob orientação da Dr.ª Ivone Rebelo, no período de 2 de maio a 30 de agosto. O estágio representa um fator importantíssimo na consolidação de conhecimentos, bem como, no paralelismo entre o Mestrado Integrado em Ciências Farmacêuticas (MICF) e a realidade profissional. No presente documento encontrase a minha reflexão desse percurso sob o conceito SWOT (Strengths, Weaknesses, Opportunities, Threats). O consumo cada vez mais diversificado de produtos alimentares aumenta o risco de interações alimentares com fármacos. As alterações alimento-fármaco alteram sobretudo a farmacocinética dos fármacos e consequentemente a sua ação normal. Os anti-inflamatórios são dos fármacos mais consumidos em Portugal e a nível Mundial, usados na redução da inflamação, da dor e da febre. O uso de anti-inflamatórios não esteroides (NSAIDs) pode provocar efeitos ao nível gastrointestinal pelo seu carater acídico e pela inibição das prostaglandinas e por isso várias entidades a nível mundial recomendam não consumir NSAIDs em estado de jejum. Contudo, para situações esporádicas aceita-se que se pode tomar em jejum para obter um efeito mais rápido. Novos conceitos surgem no mercado farmacêutico, tal como os biopotenciadores. Os biopotenciadores são substâncias que em combinação com fármacos aumentam a sua biodisponibilidade e por isso, permite a diminuição de doses e com isso diminuir efeitos adversos, toxicidade e até mesmo o custo de terapêuticas. A piperina e a curcumina demonstram ser bons biopotenciadores do ibuprofeno e do celecoxib respetivamente. Por outro lado, o sumo de toranja (GFJ) é um dos agentes com maior interferência ao nível da farmacocinética de fármacos diminuindo a efetividade das terapêuticas. O GFJ é inibidor de várias enzimas do citocromo P450 (CYPs), da P-glicoproteina (P-Gp) e até de Polipéptidos Orgânicos Transportadores de Iões (OATPs). Estas ações do GFJ demonstraram reduzir a biodisponibilidade do paracetamol bem como da pirfenidona, um recente agente anti-inflamatório. O farmacêutico desempenha um papel importante na identificação de potenciais interações, bem como, na definição de recomendações e aconselhamento de forma a assegurar a efetividade e segurança das terapêuticas aquando à toma concomitante ou não de fármacos com alimentos
The internship is the finish of academic course of 5 years. My internship took place at Monte Formoso Pharmacy, under the guidance of Dr.ª Ivone Rebelo, from May 2nd to August 30th. The internship represents a very important factor in the consolidation of knowledge, as well as in the parallelism between the Integrated Master in Pharmaceutical Sciences (MICF) and the professional reality. In this document, it is my reflection on this course under the SWOT concept (Strengths, Weaknesses, Opportunities, Threats). The higher diversified of food products consumption increases the risk of food-drug interactions. Food-drug interactions mainly occur at drug pharmacokinetics consequently their normal action is modified. Anti-inflammatory medicines are the drugs most consumed in Portugal and worldwide, used to reduce inflammation, pain and fever. The use of Nonsteroidal antiinflammatory drugs (NSAIDs) can cause gastrointestinal effects due to their acidic character and the inhibition of prostaglandins and therefore several entities worldwide recommend not to take NSAIDs in fasting. However, for sporadic situations it is accepted that for a faster effect, NSAIDs could be taken before eating. New concepts arise in the pharmaceutical market, such as bioenhancers. Bioenhancers are substances that in combination with drugs increase their bioavailability and therefore, allows the reduction of doses and thereby decrease adverse effects, toxicity and even the cost of therapeutics. Piperin and curcumin are shown to be good bioenhancers of ibuprofen and of celecoxib, respectively. On the other hand, grapefruite juice (GFJ) is an agent with greater interference in the pharmacokinetics of drugs, decreasing the effectiveness of the therapeutics. GFJ is an inhibitor of several cytochrome p450 (CYPs), P-glycoprotein (P-Gp) and even organic aniontransporting polypeptide (OATPs). These GFJ actions have been shown to reduce the bioavailability of paracetamol as well as pirfenidone, a recent anti-inflammatory agent. The pharmacist plays a significant role in the identification of potential interactions and in the definition of recommendations and counseling to ensure the effectiveness and safety of the therapeutics when take drugs with or not concomitant food.
