Abstract: Glucagon-like peptide 1 (GLP-1) is a crucial hormone that binds to its receptor (GLP-1R) and plays several beneficial roles, being is main function potentiating insulin secretion inpancreatic β cells, induced by the entry of nutrients (''incretin effect''). Thus, GLP-1R agonists(GLP-1RAs) correspond to one of the main therapeutic targets for the treatment of DMT2,such as Liraglutide, since, in addition to insulin secretion, they also promote the increase ofproliferation and the decrease of the apoptotic rate of β cells of the islets of Langerhans, andexhibit positive actions in the cardiovascular system.DMT1 occurs when the pancreas is completely unable to produce insulin or producesin low quantity, due to the total or partial absence of β cells, respectively, contributing to themaintenance of the chronic hyperglycemic state, which has as consequence several side effects,mainly endothelial dysfunction and consequently the onset of cardiovascular diseases.Therefore, the application of GLP-1RAs to DMT1 therapy, combined with insulin, is suggestedbecause of its possible beneficial effects at cardiovascular level.The main objective of this study was to evaluate the effects of Liraglutide (Victoza®) onthe cardiovascular system and how this effect is altered in the presence of DMT1.For this, DMT1 was induced with Streptozotocin (STZ) in male Wistar rats at 12 weeksof age, by single intraperitoneal injection (65 mg/kg in 10 mM citrate buffer, pH 4.5). At 14weeks, the animals were divided into four groups: two control groups, one group subject totreatment with Liraglutide (200 μg/kg x 2/day) and one not, and two diabetic groups underthe same conditions as the control groups. After 2 weeks of treatment, the animals weresacrificed for the realization of functional and biochemical studies.In the in vivo studies, the ACh induced a relaxation that was practically abolished by thepresence of NG-Nitro-L-Arginine Methyl Ester (L-NAME), verifying that the endothelium wasfunctional and the relaxation was NO-dependent. In addition, it was also demonstrated thatLiraglutide affected the vasodilatory response of ACh.For the pharmacological characterization of Liraglutide, concentration-response curveswere performed in the presence of the drug, with pre-incubation with L-NAME, Exendin-3(Ex-3) and insulin. Liraglutide was found to have a NO-independent contractile action verifiedby the non-altered response in the presence of L-NAME; this action is partially mediated byGLP-1R, since the presence of Ex-3 in the control group led to a decrease in the contractileresponse of the drug, however in the diabetic group the same was not verified. The presence of insulin leads to a decreased of the potency of the Liraglutide-induced contraction, whereasin the diabetic group the presence of insulin showed a tendency to increase the maximumLiraglutide contraction. Finally, the effect of Liraglutide and insulin was evaluated individually,with the two drugs inducing contraction, which is higher in diabetic animals.To study the mechanisms underlying the results obtained by the functional studies,biochemical studies were performed, such as the Western Blotting technique that allowed todo the quantification of GLP-1R, eNOSp / eNOS and insulin receptor (IRp/IRβ) density. GLP-1R was shown to be decreased in the non-treated diabetic group, however, in diabetic animalstreated with Liraglutide, the density was increased with a statistically significant difference (P<0.05), compared to the first group. Regarding to eNOS and its phosphorylated form, nostatistically significant differences were observed between the groups; however, the controlgroup treated with Liraglutide has demonstrated a tendency to increase the active form of theprotein. Concerning to the quantification of the IR density, both forms of the receptor aredecreased in the presence of the disorder, and treatment with Liraglutide in the control grouphas shown a tendency for impairment of the active form of the receptor.Analyzing histochemistry, there was possible to notice some changes in the aortictissue in the diabetic group, since they presented matrix alterations and tissue disorganization.Relatively to GLP-1R immunohistochemistry, it was verified that there was a heterogeneousdistribution within each group and a decrease of labeling in diabetic animals relative to controlanimals.Based on this study, we conclude that both drugs induce cardiovascular contraction,with Liraglutide contraction being NO-independent and partially mediated by its binding tothe receptor (GLP-1R). Thus, a combination therapy of insulin and Liraglutide may be beneficialin the treatment of early-stage DMT1, but possibly not in patients with a known cardiovascularhistory.
O glucagon-like peptide 1 (GLP-1) é uma hormona crucial que se liga ao seu recetor(GLP-1R) e desempenha inúmeras funções benéficas no organismo, tendo como principalfunção potenciar a secreção de insulina a nível das células β pancreáticas, induzida pela entradade nutrientes (‘’efeito incretina’’). Deste modo, os agonistas do GLP-1R (GLP-1RAs)correspondem a um dos principais alvos terapêuticos para o tratamento da DMT2, como é ocaso do Liraglutido, uma vez que, para além da promoção da secreção de insulina, tambémaumentam a proliferação e diminuem a taxa apoptótica das células β dos ilhéus de Langerhans,e exibem ações positivas no sistema cardiovascular.A DMT1 ocorre quando o pâncreas é incapaz de produzir insulina ou produz em baixaquantidade, devido á inexistência total ou parcial de células β, respetivamente, contribuindopara a manutenção de um estado hiperglicémico crónico, que tem como consequênciadiversos efeitos colaterais, principalmente a disfunção endotelial e consequente aparecimentode doenças cardiovasculares. Assim, é sugerida a aplicação de GLP-1RAs na terapêutica daDMT1, conjuntamente com a insulina, devido aos seus possíveis efeitos benéficos a nívelcardiovascular.O objetivo principal deste estudo consistiu em avaliar os efeitos do Liraglutido(Victoza®) no sistema cardiovascular e de que forma este efeito está alterado na presença daDMT1.Para tal, induziu-se DMT1 com Streptozotocina (STZ) em ratos Wistar machos com 12semanas de idade, através de uma injeção intraperitoneal única (65 mg/Kg em 10 mM detampão citrato, pH 4,5). Às 14 semanas, os animais foram divididos em quatro grupos: doisgrupos controlo, um sujeito ao tratamento com o Liraglutido (200 μg/kg X2/dia) e outro não,e dois grupos diabéticos nas mesmas condições que os grupos anteriores. Ao fim de 2 semanasde tratamento, os animais foram sacrificados, para a realização de estudos funcionais ebioquímicos.Nos estudos in vivo a ACh induziu um relaxamento que era praticamente abolido napresença de Éster Metílico de NG-Nitro-L-arginina (L-NAME), verificando-se que o endotéliose encontrava funcional e que o relaxamento era NO-dependente. Para além disso, foidemonstrado também que o Liraglutido afetava a resposta vasodilatadora da ACh.Para a caracterização farmacológica do Liraglutido foram realizados ensaios napresença do fármaco, com pré-incubação de L-NAME, Exendin-3 (Ex-3) e insulina. Verificou-se que o Liraglutido tinha uma acção contrátil NO-independente, verificada pela não alteraçãoda resposta na presença de L-NAME; esta ação é parcialmente mediada pelo GLP-1R, uma vezque a presença de Ex-3 no grupo controlo, levou a uma diminuição da resposta contrátil dofármaco, no entanto, no grupo diabético o mesmo não foi verificado. A presença de insulinano grupo controlo levou à diminuição da potência da contração induzida pelo Liraglutido,enquanto que, no grupo diabético levou ao aumento da tendência para a contração máximado Liraglutido. Avaliou-se por fim o efeito do Liraglutido e da insulina individualmente,verificando-se que os dois fármacos induzem contração, estando esta aumentada nos animaisdiabéticos.Para estudar os mecanismos subjacentes aos resultados obtidos pelos estudos funcionais, foram realizados estudos bioquímicos, nomeadamente a técnica de Western Blotting que permitiu quantificar a densidade do GLP-1R, da eNOSp/eNOS e do recetor da insulina (IRp/IRβ).O GLP-1R apresentou-se diminuído no grupo diabético não sujeito a tratamento, no entanto, nos animais diabéticos tratados com o Liraglutido a densidade encontrava-se aumentada, com diferença estatisticamente significativa (P<0,05) comparativamente ao primeiro grupo. Relativamente à eNOS e à sua forma fosforilada não foram observadas diferenças estatisticamente significativas entre os diversos grupos, no entanto, o grupo controlo tratado com Liraglutido apresentou uma tendência para o aumento da forma fosforilada da proteína. Relativamente à quantificação da densidade do IR, ambas as formas do recetor estão diminuídas na presença da patologia, e o tratamento com o Liraglutido no grupo controlo demonstrou tendência para o comprometimento da forma ativa do recetor.Recorrendo à histoquímica, foi possível detetar modificações no tecido aórtico nogrupo diabético, uma vez que se verificaram alterações matriciais e desorganização do tecido.Quanto à marcação do GLP-1R por imunohistoquímica, observou-se uma distribuição heterogénea dentro de cada grupo e uma diminuição da marcação nos animais diabéticos relativamente aos controlo. Com base neste estudo, concluímos que ambos os fármacos induzem contração a nível cardiovascular, sendo que a contração do Liraglutido é NO-independente e parcialmente mediada pela sua ligação ao GLP-1R. Assim, uma terapia conjunta de insulina e Liraglutido poderá ser benéfica no tratamento da DMT1 num estadio inicial, mas possivelmente não será a mais indicada para doentes com histórico clínico cardiovascular.
