Abstract: Dada a tendência atual para o envelhecimento progressivo das populações e sendo a síndrome de fragilidade a expressão mais problemática do envelhecimento, este é um tema com baste interesse na atualidade. Para além disto, esta síndrome é caracterizada por piores desfechos clínicos o que acarreta elevados custos para o sistema nacional de saúde. Para a realização deste trabalho foram pesquisados artigos de investigação e de revisão nas bases de dados eletrónicas PubMed, B-On e Google Académico, selecionados de acordo com o objetivo do trabalho. A fragilidade é atualmente definida com uma síndrome clínica caracterizada por aumento da vulnerabilidade à inadequada resolução ou reposição da homeostasia depois de um evento causador de stress, ou seja, é um estado de elevada suscetibilidade a consequências adversas para a saúde, como incapacidade, dependência, quedas e mortalidade. Apesar disto, nenhuma definição definitiva e consensual foi até hoje encontrada, pelo que, as mais utilizadas são a proposta por Fried et al. e a do modelo de acumulação de défices. A prevalência real desta síndrome é difícil de estabelecer devido à utilização de diferentes critérios e às diferentes características dos grupos de doentes em estudo, o tem por base a inexistência de uma definição clara e universal do conceito de Síndrome de Fragilidade.Esta síndrome apresenta uma etiologia complexa e multifatorial, que inclui variações interpessoais e do ambiente envolvente de cada idoso, pelo que, apesar de vários estudos mostrarem associações entre a incidência de fragilidade e certos fatores, uma relação definitiva ainda não foi estabelecida e é ainda pouco clara a razão pela qual alguns dos mesmos conduzem à fragilidade em certos indivíduos mas não em todos. Por outro lado, existem fatores de risco identificados que são essenciais para a prevenção e tratamento desta síndrome. Do ponto de vista fisiopatológico as alterações relacionadas com a idade em múltiplos sistemas são fundamentais para o desenvolvimento de fragilidade, principalmente nos sistemas musculosquelético, essencialmente através da sarcopemia, endócrino e imunológico. Também um estado de inflamação crónica parece ser essencial para o desenvolvimento desta síndrome. As manifestações clínicas são reconhecidas pelo médico pela conjugação de sinais e sintomas específicos como, perda de peso, diminuição da força, cansaço, lentificação motora e diminuição da atividade física. Esta síndrome está fortemente associada à existência concomitante de múltiplas outras patologias, sendo as mais importantes a hipertensão arterial e a doença coronária por terem impacto reconhecido na severidade da síndrome da fragilidade e na sobrevivência. Para o diagnóstico diferentes modelos foram propostos, como os critérios de Fried, o modelo de acumulação de défices e a escala de fragilidade de Edmonton. Todos parecem ter capacidades semelhantes na deteção de fragilidade e previsão de reações adversas, no entanto, todos apresentam algumas limitações, pelo que, nenhum método foi ainda definido para deteção de forma universal desta síndrome. Relativamente ao impacto da síndrome de fragilidade nos cuidados de saúde, sabe-se que estes doentes estão em risco de piores desfechos após um internamento ou intervenção cirúrgica, assim, a medição da fragilidade deverá integrar a avaliação global do risco cirúrgico.Para o tratamento, sabe-se atualmente que o gold standard terapêutico de doentes com síndrome de fragilidade é uma avaliação geriátrica integrada (CGA), que em conjunto com programas de exercício físico são os únicos métodos que mostraram ter impacto positivo no tratamento e evolução da síndrome de fragilidade.
Given the current trend towards the progressive aging of populations and the fact that frailty syndrome is the most problematic expression of aging, this is a topic of great interest today. In addition, this syndrome is characterized by worse clinical outcomes, which entails high costs for the national health system.For the accomplishment of this work, research articles and reviews were searched in the electronic databases PubMed, B-On and Google Scholar and selected according to the objective of the work.Frailty is currently defined as a clinical syndrome characterized by increased vulnerability to inadequate resolution or replacement of homeostasis after a stressful event, in other words, it is a state of high susceptibility to adverse health outcomes such as disability, dependence, falls and mortality. Despite that, no definitive and consensual definition has been found so far, so the ones most commonly used are those proposed by Fried et al. and that of the Frailty Index. The real prevalence of this syndrome is difficult to establish due to the use of different criteria and different characteristics of the patient groups in the studies, mainly because there is no clear and universal definition of Frailty Syndrome.This syndrome has a complex and multifactorial etiology, including interpersonal and the surrounding environment variations of each elderly person. Therefore, although several studies show associations between the incidence of frailty and certain factors, a definitive relationship has not been established yet. It is unclear why some factors lead to frailty in some but not all individuals. On the other hand, there are identified risk factors that are essential for the prevention and treatment of this syndrome.From the pathophysiological point of view, the age-related changes in multiple systems are fundamental for the development of fragility, especially in the musculoskeletal, essentially through sarcopenia, endocrine and immunological systems. In addition, a state of chronic inflammation seems to be essential for the development of this syndrome.Clinical manifestations are recognized by the physician by the combination of specific signs and symptoms such as weight loss, decreased strength, fatigue, slowness and decreased physical activity. This syndrome is strongly associated with the existence of multiple other pathologies, the most important of which are arterial hypertension and coronary heart disease, because they have great impact on the severity of the frailty syndrome and in the survival.For the diagnosis, different models were proposed, such as the Fried’s criteria, the cumulative deficit model and the Edmonton frailty scale. All seem to have similar abilities in the detection of fragility and prediction of adverse outcomes, however all present some limitations, reason why, no method was still defined for universal detection of this syndrome.Regarding the impact of the fragility syndrome on health care, it is known that these patients are at risk for worse outcomes after hospitalization or surgical intervention, so the measurement of fragility should be part of the overall assessment of surgical risk.For the treatment, the gold standard for the management of patients with frailty syndrome is the Compressive Geriatric Assessment (CGA), which together with physical exercise programs are the only therapeutic methods that have shown to have a positive impact on treatment and evolution of this syndrome.
