dc.description.abstract | Introdução: O uso da terapêutica com perfusão subcutânea contínua de insulina (PSCI) na diabetes mellitus tipo 1 (DM1) é cada vez mais frequente devido aos seus efeitos benéficos no controlo glicémico.Objetivo: Avaliar o impacto da PSCI no controlo glicémico, peso, índice de massa corporal (IMC), dose diária total de insulina (DDTI) e complicações desta modalidade terapêutica durante vinte anos de seguimento.Métodos: Este estudo retrospetivo inclui doentes com DM1 que iniciaram terapêutica com PSCI até 2005, seguidos no Centro Hospitalar e Universitário de Coimbra (CHUC). Foi avaliado a hemoglobina glicada A1c (HbA1c), peso, IMC, DDTI e frequência de complicações associadas à PSCI imediatamente antes e 6 meses, 1 ano, 5, 10, 15 e 20 anos após PSCI a partir dos registos médicos das consultas de seguimento no CHUC.Resultados: Obtiveram-se dados de 26 doentes (7 homens; 19 mulheres) com duração de doença até início da PSCI de 17,73±7,92 anos, idade de início de PSCI de 31,92±8,75anos e seguimento durante 13,12±2,07 anos. Os principais motivos para colocação de bomba foram: inadequado controlo metabólico em 80,8%, história de hipoglicemias assintomáticas ou severas em 15,4%, e gravidez/planeamento de gravidez em 3,8% dos casos. A HbA1c média prévia foi 9,5±2,3% tendo diminuído aos 6 meses para o valor mínimo de 7,3±1,1% (p=0,0003). Verificou-se uma diminuição estatisticamente significativa da HbA1C apenas nos primeiros 10 anos de seguimento.Não se verificou nenhuma diferença estatisticamente significativa na variação do peso ao longo de todo o seguimento. No IMC obteve-se uma diferença estatisticamente significativa aos 5 anos de seguimento comparativamente com o IMC prévio à PSCI (p=0,009).As necessidades diárias de insulina reduziram-se de 60,6±16,7 U/dia para 44,9±10,6 U/dia (p=0,005) nos primeiros 6 meses e não se encontrou diferenças estatísticas no restante seguimento.Verificaram-se 4 hipoglicemias severas (incidência 0,015/doente/ano), 6 cetoacidoses diabéticas (0,022/doente/ano) e nenhuma infeção no local de inserção do cateter.Discussão e Conclusão: Este estudo demonstrou a eficácia da PSCI que está associada a uma diminuição significativa da HbA1c sustentada durante 10 anos e a uma redução da DDTI, significativa nos primeiros 6 meses. A taxa de complicações agudas foi baixa e semelhante à da literatura existente. | por |
dc.description.abstract | Introduction: The use of continuous subcutaneous insulin infusion (CSII) therapy in type 1 diabetes mellitus (T1DM) has increased due to its benefits on glycemic control.Aim: To assess the impact of CSII on glycemic control, weight, body mass index (BMI), total daily dose of insulin (TDDI) and complications associated with this therapy, during 20-year follow-up.Methods: This retrospective study included patients with T1DM who started CSII therapy up until 2005, followed at Centro Hospitalar e Universitário de Coimbra (CHUC). Glycated hemoglobin A1c (HbA1c), weight, BMI, DDTI and acute complications associated with CSII were evaluated immediately prior to initiation of CSII and 6 months, 1 year, 5, 10, 15 and 20 years after CSII.Results: This study included twenty-six patients (7 males, 19 females) with mean disease duration up to the start of CSII of 17.73±7.92 years, age of onset of CSII of 31.92±8.75 years and follow-up during 13.12±2.07 years. The main reasons for starting pump therapy were: inadequate metabolic control in 80.8%, history of asymptomatic or severe hypoglycemia in 15.4%, and pregnancy/pregnancy planning in 3.8% of the cases. The mean HbA1c was 9.5±2.3% and, at 6 months, decreased for the minimum value of 7.3±1.1% (p=0.0003). There was a statistically significant decrease in HbA1c only in the first 10 years of follow-up.There was no statistically significant difference in weight variation over all follow-up. Relative to BMI, a statistically significant difference was observed at the 5-year follow-up compared to the BMI prior to the CSII (p=0.009).Daily insulin requirements were reduced from 60.6±16.7U/day to 44.9±10.6U/day (p=0.005) at 6 months and no statistical differences were found in the remaining follow-up.There were 4 severe hypoglycemia (incidence 0.015/patient/year), 6 diabetic ketoacidosis (0.022/patient/year) and no infection at the site of catheter insertion.Discussion and Conclusion: This study shows that CSII therapy improved glycemic control, especially during the first 10 years of follow-up and allowed a significant decrease in DDTI in the first 6 months. The rate of acute complications were low and similar to that of the existing literature. | eng |
