dc.description.abstract | A terapêutica fotodinâmica requer três elementos: a luz, o fotossensibilizante e o oxigénio. A reação fotodinâmica leva a destruição celular duplamente seletiva, já que a fonte de luz é direcionada para a lesão e porque o fotossensibilizante é preferencialmente absorvido pelas células em proliferação. É uma técnica bem tolerada, pouco invasiva e com excelentes resultados cosméticos. É limitada principalmente pela capacidade de absorção do fotossensibilizante e pela dor durante a irradiação. Atualmente estão disponíveis os fotossensibilizantes ALA, MAL e BF-200 ALA aprovados para uso nas queratoses actínicas, carcinoma basocelular e espinhocelular in situ. Mais recentemente, as aplicações experimentais da terapia fotodinâmica têm sido expandidas para o tratamento e prevenção de cancro cutâneo não-melanoma em doentes transplantados, o fotoenvelhecimento, acne, verrugas, micose fungóide, morfeia, psoríase, leishmaniose, entre outras. Neste trabalho foi feita uma revisão das aplicações atuais e potenciais da terapêutica fotodinâmica, baseada numa pesquisa da literatura científica. Conclui-se que o potencial desta técnica em dermatologia é imenso, sobretudo à medida que surgem novos fotossensibilizantes e novas fontes de luz, sendo que é esperado no futuro o alargamento das suas indicações. dc.description.abstract | Photodynamic therapy is based on three elements: light, a photosensitizer and oxygen. The photodynamic reaction elicits cellular destruction with dual selectivity, as a result of the light source being directly targeted on the lesional tissue and because the photosensitizer is preferentially absorbed by hyperproliferative cells. It is a well-tolerated technique, non-invasive and with excellent cosmetic outcomes. Its major limitations are the capacity of penetration of the photosensitizer and the pain during irradiation.The current approved applications in dermatology for the photosensitizers ALA, MAL and BF-200 ALA are actinic keratosis, basal cell carcinoma and squamous cell carcinoma in situ. Recently the experimental uses have been expanded to the treatment and prevention of non-melanoma skin cancer in organ-transplant recipients, photorejuvenation, acne, warts, leishmaniasis, mycosis fungoides, psoriasis and others.In this article we review the current and potential applications of photodynamic therapy, based on the research of the scientific literature. We conclude that this technique has a huge potential in dermatology, and it is expected that the approved applications will be broaden, mainly if new, more efficient photosensitizers and light sources are developed.
