dc.description.abstract | As perturbações neurocognitivas associadas ao vírus da imunodeficiência humana [em inglês, HIV- associated neurocognitive disorders (HAND)] são um importante fator de morbilidade e mortalidade nos doentes VIH positivos. A sua apresentação clínica mudou de paradigma com a instituição da terapia cART, tendo a sua incidência e severidade diminuído. No entanto, as formas mais moderadas da patologia são ainda extremamente comuns, atingindo cerca de metade da população infetada por VIH.A neuropatogénese desta patologia está ainda longe de ser totalmente compreendida. O VIH entrará no SNC nas fases inicias da infeção, e a consequente inflamação e ativação imune persistem no decorrer da doença, contribuindo, muito provavelmente, para o desenvolvimento da HAND. Considerando a importância clínica da doença, a elevada prevalência e falta de terapêutica dirigida, é necessária mais investigação sobre o tema.Neste trabalho apresentou-se uma revisão do estado da arte relativa à neuropatogénese, complicações neurológicas, fatores de risco, meios de diagnóstico e tratamento, considerando tanto as atuais terapêuticas antirretrovirais como possíveis terapias adjuvantes. Discutiu-se ainda a possibilidade de erradicação da carga viral VIH, refletindo no papel do SNC como reservatório viral, o que permitirá a progressão da replicação do VIH mesmo em casos de supressão viral sistémica eficaz. ..................................................................................................................................... | por |
dc.description.abstract | HIV associated cognitive disorders (HAND) are a major cause of morbidity and mortality within HIV-seropositive population. Though the observed decrease on its severity and prevalence, by the implementation of combination antiretroviral therapy (cART), the mild forms of the disease continue to prevail, concerning half of the patients with HIV infection. The neuropathogenesis is far from being fully understood. Evidence suggests that HIV enters the brain in the early stages of the infection, and persistent inflammation and immune activation of the CNS presumably contribute to the development of HAND.Given evidence of the clinical impact of the disease, high prevalence, and lack of specific therapeutics, future investigation is needed.In this review, we discuss the neuropathogenesis, clinical manifestations, host risk factors, diagnosis and therapeutics including both antiretroviral therapy and prospective direct strategies.
