Abstract: Introdução: A hipertensão intracraniana idiopática (HII) é caracterizada por aumento da pressão intracraniana de causa desconhecida. A sua incidência varia entre 1 e 3/100.000/ano, tendo uma maior prevalência em mulheres obesas em idade fértil (12-28/100.000/ano). Espera-se que esta aumente, tendo em conta a panorâmica da obesidade mundial. Portugal tem também seguido o padrão crescente da obesidade, daí a importância do conhecimento por parte dos profissionais de saúde desta patologia.Material e Métodos: Foram selecionados artigos retirados de bases de dados Cochrane, PubMed e Embase. Os termos de pesquisa utilizados foram: “pseudotumor cerebri”, “etiology”, “physiopathology”, “diagnosis”, “therapeutics”, “surgical procedures, operative”, tendo a pesquisa sido finalizada a 7 de novembro de 2017. Os limites utilizados na pesquisa foram: data de publicação dos artigos, entre 2007 e 2017; língua das publicações, inglês e português e estudo realizados em humanos, com idade superior a 19 anos.Resultados: A etiologia e a fisiopatologia ainda são um tema de muita controvérsia, não havendo certezas acerca dele. Neste artigo são apresentadas algumas das possibilidades colocadas. O diagnóstico é efetuado com base nos critérios de Dandy modificados, com o auxílio de métodos complementares de diagnóstico, sendo os parâmetros observados em cada um descritos nesta revisão. A terapêutica ainda não se encontra uniformizada. Neste trabalho são abordadas as diferentes possibilidades terapêuticas que se podem utilizar nesta doença, bem como as indicações principais de cada uma.Discussão e Conclusão: Há vários mecanismos envolvidos na fisiopatologia desta doença, podendo esta tratar-se de uma doença multifatorial, na qual o diagnóstico já se encontra bem sedimentado. O Idiopathic Intracranial Hypertension Treatment Trial (IIHTT) veio consolidar a utilização da acetazolamida associada a perda de peso como a primeira linha terapêutica nos doentes com HII, estando as opções cirúrgicas reservadas para casos de HII fulminantes ou para situações nas quais houve um agravamento da sintomatologia, sendo estas refratárias à terapêutica médica. Foi reconhecida a importância de todos os doentes com HII realizarem um seguimento prolongado, devido ao risco de terem recorrência ou agravamento das alterações visuais. Tendo em conta a falta de significância estatística e a variabilidade de muitos dos resultados, devido a amostras pequenas e à utilização de diferentes metodologias, proponho a realização de um estudo multicêntrico, com metodologias estandardizadas. Este tem como finalidade a obtenção de resultados representativos da população por meio duma amostragem significativa e a aquisição duma base de dados epidemiológica dos doentes com HII das diferentes zonas do globo. Atualmente, encontram-se em execução diversos estudos relativos à HII. É importante reter que, tendo em conta a morbilidade que lhe está associada, torna-se imperativo o conhecimento desta patologia e o seu diagnóstico precoce, com vista a um tratamento atempado, de modo a impedir um desfecho dramático.
Introduction: Idiopathic intracranial hypertension (IIH) is characterized by raised intracranial hypertension of unknown cause. Its worldwide incidence ranges from 1 to 3/100.000 per year, with a higher incidence in obese women of childbearing age (12-28/100.000/year). It is expected to increase, considering an overview of global obesity. Portugal has also followed the increasing pattern of obesity, hence the importance of the knowledge of this pathology by the health professionals.Methods: There have been selected articles researched on the data bases Cochrane, PubMed and Embase. The research terms used were: “pseudotumor cerebri”, “etiology”, “physiopathology”, “diagnosis”, “therapeutics”, “surgical procedures, operative”, being the research finalized at November 7, 2017. The filters used in the research were: publication dates, between 2007 and 2017, language of the publications, English and Portuguese and study carried out in humans with age higher than 19 years.Results: Etiology and pathophysiology are still a subject of much controversy, since there are no certainties about it. This article presents some possibilities. The diagnosis is made based on modified Dandy criteria, with the aid of complementary diagnostic methods, being the parameters observed in each one described in this review. Disease management is not yet standardized. In this work is discussed the different therapeutic possibilities that can be used in this disease, as well as the main indications of each one.Discussion and Conclusion: There are several mechanisms involved in the pathophysiology of this disease, which may be a multifactorial disease, in which the diagnosis is already well established. The Idiopathic Intracranial Hypertension Treatment Trial (IIHTT) has consolidated the use of acetazolamide associated with weight loss as the first therapeutic step in patients with IIH, with surgical options reserved for cases of fulminant HII or for situations in which there was an aggravation of the symptomatology, which are refractory to medical therapy. The importance of a long-term follow-up for all patients with IIH was recognized, given the risk of recurrence or worsening of visual impairment. Given the lack of statistical significance and the variability of many of the results, due to small samples and the use of different methodologies, I propose the elaboration of a multicenter study, with standardized methodologies. The aim of this study is to obtain representative population results by means of a significant sampling and acquisition of an epidemiological database of patients with IIH from different parts of the globe. Several HII studies are currently under way. It is important to note that, given the associated morbidity, it is imperative to know this pathology and to make an early diagnosis, with a view to a timely treatment, in order to prevent a dramatic outcome.
