A elevada mortalidade e morbilidade associada fez com que a vacinação anti-pneumocócica se tornasse numa das mais importantes medidas de saúde pública. Duas vacinas estão atualmente disponíveis para imunização do idoso: a vacina pneumocócica polissacarídica de 23 valências (PPV23) e a vacina pneumocócica conjugada de 13 valências (PCV13). A PPV23 foi eficaz na prevenção de doença invasiva pneumocócica, embora o seu benefício na prevenção de pneumonia permaneça controverso. A sua eficácia foi limitada, com curta duração de proteção, sendo inferior nos mais idosos e na presença de comorbilidades. A PCV13, além de ser mais imunogénica, apresentou maior benefício clínico, por redução da incidência de doença pneumocócica, da colonização do idoso e da resistência ao tratamento médico. Ambas as vacinas apresentaram um bom perfil de segurança, com reações auto-limitadas. A resposta imunológica após revacinação com PPV23 foi imunogénica, mesmo nos mais idosos, apesar de ser inferior à observada pós-primovacinação. Contudo, a associação de ambas as vacinas demonstrou ser a opção que conferiu maior grau de imunidade, com menos efeitos adversos que revacinar com PPV23.O efeito de imunidade de grupo, alienada à emergência de serotipos não-vacinais e à elevada prevalência de serotipos da PPV23 em circulação, como o observado em Portugal, pode obviar a necessidade de revisão das recomendações de vacinação anti-pneumocócica atuais. The natural decline of the immune system places the elderly at risk of pneumococcal infection. The high morbidity and mortality associated with such an infection made the pneumococcal vaccination one of the most important measures in public health. Two vaccines are currently available for immunization in the elderly: 23-valent pneumococcal polysaccharide vaccine (PPV23) and 13-Valent Pneumococcal Conjugate Vaccine (PCV13). PPV23 was effective in preventing invasive pneumococcal disease, although its benefit in preventing pneumonia remains controversial. Its effectiveness was limited, with short-term duration of protection, being lower in the elderly population and in the presence of comorbidities. The PCV13, despite being more immunogenic, also showed a greater clinical benefit, by reducing the incidence of pneumococcal disease, colonization in the elderly, and resistance to medical treatment. Both vaccines presented a good safety profile, with self-limited reactions. The immunological response after revaccination with PPV23 was immunogenic, even in the very elderly, although it was lower than the observed after primary vaccination. However, the association of both vaccines was shown to be the option that conferred the highest degree of protection, with fewer adverse effects than revaccinating with PPV23.Herd immunity, associated with the increase in non-vaccine strains and the high prevalence of serotypes of PPV23 in circulation, such as observed in Portugal, may imply the need for revision of current pneumococcal vaccination recommendations. |
