dc.description.abstract | Introdução: A doença hepática crónica apresenta elevada prevalência e o consumo crónico de álcool estima-se que seja responsável por cerca de 30% dos casos de doentes submetidos a transplante hepático (TH). Até cerca de 70% dos doentes com cirrose hepática apresentam sarcopenia, que pode ser definida como a perda progressiva de massa e, consequentemente, de força muscular. A avaliação da sarcopenia pode ser feita de forma objetiva com recurso a várias modalidades, incluindo a tomografia computorizada (TC). A sarcopenia reflete o estado nutricional do doente e está identificada em determinadas áreas como facto de prognóstico independente, condicionando maior morbimortalidade.Objetivo: avaliar a existência de sarcopenia e a sua relação com a evolução analítica no pós-operatório imediato de doentes submetidos a TH.Material e Métodos: foram avaliados, retrospetivamente, todos os doentes submetidos a TH na Unidade de Transplantação Hepática Pediátrica e de Adultos do Centro Hospitalar e Universitário de Coimbra, entre 1 de Janeiro de 2015 e 31 de Dezembro de 2017. Após a aplicação de critérios de inclusão, foi avaliada a existência de sarcopenia com base no cálculo da Área do Músculo Psoas em TC efetuada no estudo pré-TH, permitindo obter 2 grupos de doentes (sarcopénicos versus não-sarcopénicos). Múltiplas variáveis do doente, dados do período peri-TH e dados analíticos, foram posteriormente avaliados e submetidos a estudo estatístico.Resultados: do total de 211 doentes inicialmente incluídos, apenas 50 cumpriam os critérios de inclusão. O estudo estatístico não revelou diferenças entre grupos relativamente a dados antropométricos, dias de terapêutica antibiótica ou antifúngica, dias de ventilação mecânica invasiva ou dias de internamento. No entanto, alguns parâmetros analíticos como a transaminase glutâmica oxalacética (p=0.014) e a bilirubina total (p=0,045) ao 7º dia e a gama glutamil transpeptidase (p=0,033) ao30º dia pós-TH apresentam diferença estatisticamente significativa, sendo que os doentes sarcopénicos apresentam valores mais baixos.Conclusão: Este trabalho sugere que, num grupo específico de doentes, a sarcopenia pode estar associada a uma melhor evolução analítica e, consequentemente, melhor outcome a longo prazo. | por |
dc.description.abstract | Background: Chronic liver disease (CLD) has high prevalence and chronic alcohol abuse is estimated to be responsible for around 30% of liver transplants (LT) worldwide. Sarcopenia is a syndrome characterized by progressive and generalized loss of skeletal muscle mass and strength with a risk of adverse outcomes such as physical disability, poor quality of life and death. Studies show that sarcopenia can be found in around 70% of patients with liver cirrhosis. There are some tests able to identify and measure sarcopenia including CT-scan. Patients nutritional status is related to sarcopenia and, in some specific areas, is considered an independent prognostic factor related with increased mortality and morbidity.Aim: Evaluate the incidence of sarcopenia and its relation with laboratory liver function tests in the post-operatory period of patients submitted to LT.Methods: We retrospectively reviewed all patients submitted to LT in Unidade de Transplantação Hepática Pediátrica e de Adultos at Centro Hospitalar e Universitário de Coimbra between January 1st, 2015 and December 31st, 2017. After patient selection regarding specific criteria, Psoas Muscle Area (PMA) was measured using pre-LTCT-scan and 2 working groups were established regarding the presence or absence of sarcopenia. Statistical analysis was then performed using data from the patient and a variety of parameters regarding the pre- and post-LT period.Results: Of all the 211 patients submitted to LT in that period, only 50 fulfilled the study criteria. Statistical analysis show no difference between sarcopenic andno-sarcopenic patients regarding their biometry, days of antibiotic and antifungal therapy, days of mechanical ventilation or days until discharge. On the contrary, some liver function tests on day-7 like aspartate aminotransferase (p=0.014) and total bilirubin (p=0,045) and on day-30 like gamma glutamyl transpeptidase (p=0,033) show statistical significance with sarcopenic patients with lower values.Conclusions: This study suggests that in this specific group of patients, sarcopenia may be related with better analytical evolution after LT and, maybe, better long term outcomes. | eng |
