dc.description.abstract | Objetivo: Avaliar as características anatómicas das queratites infeciosas obtidas com a tomografia de coerência ótica de segmento anterior (OCT-SA). Identificação de padrões específicos sugestivos do agente envolvido na queratite infeciosa. Avaliação das potencialidades do OCT-SA na orientação precoce desta patologia e melhoria do seu prognóstico.Material e Métodos: Neste estudo prospetivo e observacional, foram incluídos 24 doentes com queratite infeciosa comprovada por cultura ou prova terapêutica, que recorreram ao Serviço de Oftalmologia do Centro Hospitalar Universitário de Coimbra (CHUC). Os doentes foram submetidos a OCT-SA à entrada, todos executados pelo mesmo operador. As imagens obtidas foram então analisadas e cruzadas com os dados dos processos clínicos dos respetivos doentes. Resultados: Dos 24 doentes em estudo (14 homens e 10 mulheres), apenas 2 não apresentavam fatores de risco para queratite infeciosa (sendo os fatores de risco mais comuns o trauma ocular prévio, 11 casos, e o uso de lentes de contacto, 6 casos). Quatorze doentes revelaram infeção bacteriana, 3 infeção fúngica, 2 infeção por parasitas e 5 casos apresentaram resultado negativo para cultura microbiológica. Foram encontrados e definidos 9 padrões no OCT-SA.Conclusão: O papel do OCT-SA na identificação do agente causal é ainda limitado. Contudo, este estudo conclui que o OCT-SA complementa a biomicroscopia, permitindo uma melhor caracterização dos padrões que podem ajudar a estadear e dar indicações prognósticas acerca de quadros de queratite infeciosa. | por |
dc.description.abstract | Purpose: To evaluate the anatomical characteristics of infectious keratitis obtained with anterior segment optic coherence tomography (AS-OCT). To identify specific patterns suggestive of the pathological agent involved in the disease. To evaluate the potentialities of the AS-OCT on the early guidance of the infectious keratitis and its benefits in the prognosis.Materials and Methods: In this prospective case series and observational study, there were included 24 patients with proved infectious keratitis, either by culture or therapeutic trial, that sought medical help in the Ophthalmological Service of the Centro Hospital Universitário de Coimbra (CHUC). The patients were submitted to AS-OCT in the first contact, all executed by the same operator. The images obtained were analyzed and crossed with the patients’ data from their clinical register.Results: Of the 24 patients studied (14 men and 10 women), only 2 did not present risk factors for infectious keratitis (being the most common factors the personal history of ocular trauma, 11 cases, and the use of contact lens, 6 cases). Fourteen patients revealed bacterial infection, 3 fungal infection, 2 parasitic infection and 5 cases presented a negative microbiological culture. There were found and defined 9 morphological patterns using AS-OCT.Conclusion: The role of OCT-SA in the identification of the causative pathogen is still limited. However, this study concludes that the AS-OCT can complement the biomicrography, allowing a better characterization of the patterns that can help in the staging of the disease and giving useful prognostic information. | eng |
