Abstract: Introdução: A Tetralogia de Fallot corresponde a 7-10% de todas as cardiopatias congénitas. A sua etiologia é multifatorial e a sua evolução clínica é variável, apresentando-se geralmente com bom prognóstico após correção cirúrgica. O presente estudo teve como objetivo fazer uma análise descritiva da amostra de indivíduos portadores de Tetralogia de Fallot, cujo tratamento e follow-up clínico foi realizado no Hospital Pediátrico do Centro Hospitalar e Universitário de Coimbra. Materiais e métodos: Através de um estudo retrospetivo, baseado apenas em registos clínicos, analisaram-se e descreveram-se os dados demográficos, antropométricos, perinatais e a evolução clínica dos indivíduos de nacionalidade Portuguesa, diagnosticados com Tetralogia de Fallot e seguidos no centro hospitalar acima referido, nos últimos 18 anos. Resultados e Discussão: Cinquenta e oito indivíduos foram incluídos no estudo, com uma média de idade de 9.77 anos. Dois terços eram do sexo masculino, registando-se uma média de 3.22 novos casos por ano. O diagnóstico pré-natal foi realizado em 46.81% dos cardiopatas, a idade gestacional média foi de 38 semanas e o peso médio de nascimento foi 2856.08g. 36.21% apresentaram concomitantemente outras malformações congénitas. O z-score médio para a estatura foi de -1.01. O Índice de Massa Corporal apresentou um z-score médio de -0.35. A idade média de execução da cirurgia paliativa, realizada em 23% da amostra, foi de 2.52 meses e de 15.15 meses para a cirurgia corretiva. A mortalidade foi de 3.45%. A duração média dos internamentos necessários para estudo foi de 2.07 dias, e 3.45 dias por manifestações cardíacas. 50% necessitam de terapêutica farmacológica e 75.5% das crianças com idade igual ou superior a 6 anos inserem-se na classe I da classificação funcional da New York Heart Association.Conclusão: A população pediátrica com Tetralogia de Fallot seguida e tratada na região centro de Portugal segue as tendências demográficas, perinatais, antropométricas e de evolução clínica já descritas em literatura científica havendo, contudo, uma pequena discrepância relativamente ao número de indivíduos submetidos a intervenção paliativa. A taxa de sobrevivência a curto e médio prazo é muito boa, com indicadores de qualidade de vida bastante favoráveis.
Introduction: Tetralogy of Fallot accounts for 7-10% of all congenital cardiopaties. Its etiology is multifactorial, and the clinical evolution is variable, generally with a favorable prognostic after surgical repair. The aim of the present work was to describe a sample of paediatric Tetralogy of Fallot patients, whose treatment and clinical follow-up was conducted in the Pediatric Hospital the Coimbra Hospital and Universitary Centre, Portugal. Materials and methods: Demographical, anthropometric, perinatal and clinical data were obtained from the retrospective analysis of the clinical files of Portuguese individuals, followed in the above mentioned Hospital and diagnosed with Tetralogy of Fallot within the last 18 years. Results and discussion: Fifty-eight individuals were included in the study. Two thirds were male, and the mean age was 9.77 years. On average, there were 3.22 new cases per year. The antenatal diagnosis was performed in 46.81% of the newborns, the average gestational age at birth was 38 weeks and the mean birth weight was 2856.08g. 36.21% of the group presented had associated congenital malformations. The mean stature z-score was -1.01, with an average IMC z-score of -0.35. The palliative surgery was performed in 23% of the sample, at an average age of 2.52 months. Corrective surgery was executed at a mean age of 15.15 months. The mortality rate was 3.45%. The average hospitalization duration for complementary diagnostic exams was 2.07 days, and 3.45 for optimizing medical treatment. 50% required pharmacological therapy. 75.5% of the children aged 6 or above belonged to Class I of the New York Heart Association's functional classification.Conclusion: The paediatric population born with Tetralogy of Fallot, whose follow-up and medical treatment was performed in the central region of Portugal, presented demographic, perinatal, anthropometric and clinical data similar to those presented in the international scientific literature. However, a small discrepancy on the percentage of individuals submitted to palliation was noted. The survival rate in the short and mid-term is highly satisfactory, and the quality of life indicators are encouraging.
