Abstract: Background: Diabetes mellitus is one of the most common chronic pathologies worldwide, with the number of diabetics increasing substantially in the last decades. Adherence to non-pharmacological therapy is of great importance in the prevention of the many complications that a diabetic patient is susceptible to develop, complications that entail costs in health and worse quality of life. Therefore, one must understand the factors related to this adherence.Objective: The aim of this study is to understand if there is any relationship between empowerment, knowledge, quality of life and adherence to non-pharmacological therapy in type 2 diabetes mellitus (T2DM). Another objective will be to determine if there are any differences related to the metabolic control of T2DM and sociodemographic variables.Methods: Cross-sectional observational study in a convenience sample of patients with diabetes type 2 in a primary care unit (USF Cruz de Celas). Data, referring to 31 diabetic patients, was collected between july 6th, 2017 and December 12th, 2017. The 31 participants signed the informed consent and answered an instrument that included the Portuguese version of 5 questionnaires - DKT; DES-SF; DHP-18; EQ-5D and SDSCA and a sociodemographic and clinical characterization of the diabetics interviewed.Results: The sample consisted of 31 individuals, 51,6% of which were female. The mean age was 72.74 ± 10.10 years. The mean value of the last two measurements known of HbA1c was 6,72 ± 0.94% and the mean time of diagnosis was 12,06 ± 8,61 years. There was a significant negative correlation (p= 0,027; =-0,397) between adherence to non-pharmacological therapy (SDSCA) and psychological distress (DHP-18 SP). The remaining correlations were not statistically significant but there appeared to be a tendency for people with greater adherence to non-pharmacological therapy to have less knowledge, greater empowerment, less uninhibited food, more barriers to activity, better health-related quality of life, longer disease evolution and lower mean of the last two HbA1c measurements.Discussion and Conclusion: According to the results obtained, adherence to non-pharmacological therapy seems to be related in a negative and statistically significant way to psychological distress. This data suggests that people who are less psychologically involved tend to be more self-careful with diabetes, which alerts us to the need for greater motivational investment in people with type 2 diabetes who are in greater psychological distress. The non statistically significant correlations may be related to the limited size of the sample.
Introdução: A diabetes mellitus é uma das patologias crónicas mais comuns a nível mundial, tendo o número de diabéticos aumentado substancialmente nas últimas décadas. A adesão à terapêutica não farmacológica assume uma grande importância na prevenção das muitas complicações que a pessoa com diabetes é suscetível de desenvolver. Estas complicações acarretam custos em saúde e pior qualidade de vida deste grupo de doentes, pelo que é fundamental perceber quais os fatores relacionados com esta adesão.Objetivos: Este trabalho tem como intuito perceber se existe relação entre a capacitação, conhecimentos, qualidade de vida e adesão à terapêutica não farmacológica na diabetes mellitus tipo 2 (DM2). Outro objetivo será averiguar se existem diferenças em função do controlo metabólico da DM2 e das variáveis sociodemográficas.Material e Métodos: Foi realizado um estudo exploratório observacional e transversal em amostra de conveniência de utentes com DM2, seguidos em consulta numa Unidade de Saúde Familiar. Os 31 participantes assinaram previamente um consentimento informado e responderam a um questionário que incluía a versão portuguesa de 5 questionários – DKT; DES-SF; DHP-18; EQ-5D e SDSCA e uma caracterização sociodemográfica e clínica dos diabéticos entrevistados. Resultados: Amostra constituída por 31 inquiridos, dos quais 51,6% eram do sexo feminino. A média de idades foi 72,74 ± 10,10 anos. O valor médio das duas últimas medições de HbA1c conhecidas foi 6,72 ± 0,94% e o tempo médio de diagnóstico 12,06 ± 8,61 anos. Observou-se uma correlação negativa, estatisticamente significativa (p= 0,027; =-0,397), entre a adesão à terapêutica não farmacológica (SDSCA) e o sofrimento psicológico (DHP-18 SP). As restantes correlações não tiveram significância estatística, parecendo existir uma tendência para as pessoas com maior adesão à terapêutica não farmacológica terem menor conhecimento, maior capacitação, menos alimentação desinibida, mais barreiras à atividade, melhor qualidade de vida relacionada com a saúde, maior idade, maior tempo de evolução da doença e menor média das duas últimas medições de HbA1c.Discussão e Conclusão: De acordo com os resultados obtidos, a adesão à terapêutica não farmacológica parece relacionar-se de forma negativa e estatisticamente significativa com o sofrimento psicológico. Estes dados sugerem que as pessoas que menos sofrem psicologicamente praticam mais atividades de autocuidado com a diabetes, o que nos alerta para a necessidade de um maior investimento motivacional nas pessoas com DM2 que estão em maior sofrimento psicológico. As restantes tendências não mostraram significância estatística, o que poderá estar relacionado com o tamanho limitado da amostra, sendo necessários estudos mais alargados.
