Abstract: A impactação dentária ocorre predominantemente na dentição permanente. A evolução do processo eruptivo na direção oposta ao plano oclusal é designada dente invertido. Esta revisão sistemática pretende identificar os itens clínicos mais importantes desta patologia, nomeadamente o diagnóstico e os procedimentos terapêuticos, de acordo com o enquadramento médico-legal. A revisão foi realizada com base na metodologia PRISMA e segundo a questão PICO: Qual a conduta do médico dentista na suspeita de dentes impactados e invertidos? Dos artigos com acesso ao texto completo foram selecionados aqueles que preencheram os critérios de inclusão. Foram obtidos 372 resultados, dos quais 249 foram excluídos após avaliação do título ou resumo. Dos restantes 123 resultados, eliminámos 72 uma vez que não se enquadravam nos critérios de inclusão, deixando um total de 27 publicações. Não há registos científicos, consensuais, na enumeração de um protocolo de intervenção em relação à patologia dente invertido. A escolha da terapêutica recai na análise de cada caso per si. Na sequência da revisão sistemática foi efetuado um estudo coorte retrospetivo de onde se obteve um único caso clínico. Menina de 6 anos com patologia de dente impactado e invertido (segundo pré-molar inferior direito). Período de seguimento de 8 anos. O dente permaneceu in situ, assintomático, sem identificação de alterações nas estruturas anatómicas dentárias adjacentes, desde os 6 aos 14 anos de idade. Quando foi identificado um comprometimento severo do osso cortical e após avaliação imagiológica, procedeu-se à remoção cirúrgica do 45, sob anestesia geral e em regime ambulatório. Foi efetuada a apreciação médico-legal do caso descrito. A revisão sistemática apresentada é única no tema em estudo. A análise da erupção dentária deve ser uma das prioridades do médico dentista. Não há evidências cientificas de um protocolo consensual. Os estudos apresentados fundamentam a precocidade do diagnóstico na patologia de dente impactado e invertido. A intervenção do profissional de saúde deverá ser orientada pela monitorização clinica e imagiológica, com a apreciação médico-legal na decisão temporal e seletiva da terapêutica.
The impacted teeth occurs predominantly in the permanent dentition. The evolution of the eruptive process in the opposite direction to the occlusal plan is called inverted tooth. This systematic review intends to identify the most important clinical items in this pathology, namely the diagnosis and the therapeutic procedures in accordance with the medical and legal framework. The review was performed following PRISMA methodology and according to the PICO question, which is: What should the dentist do when suspecting an impacted and inverted tooth? Of all articles with access to the full text were just selected those who met the inclusion criteria. As a result of the analysis performed, 372 articles were identified, of which 249 were excluded after evaluating its title or abstract. Considering the remaining 123 articles, 72 were eliminated due to the fact that did not fit in the inclusion criteria, leaving a total number of 27 publications. There are neither scientific nor consensual records in the enumeration of an intervention protocol in the inverted tooth pathology. In practice, the treatment’s choice rests on the analysis of each individually case. Following the systematic review, a retrospective cohort study was conducted in which a single clinical case was obtained. A 6 years old girl with an impacted and inverted tooth (second right lower premolar). Follow up período of 8 years. The tooth remained in situ, asymptomatic and with no identified changes in the adjacent dental anatomical structures, from the ages of 6 to 14 years old. When a severe cortical bone involvement was identified and after an imaging evaluation was performed, a surgical removal of the 45 took place under general anaesthesia and in outpatient regimen. The respective medical and legal assessment of the described case was carried out. The systematic review presented is unique in terms of the subject of study. The analysis of the dental eruption should be one of the dentist's priorities. There is no scientific evidences of a consensual protocol. The presented studies support the precocity of the diagnosis in the impacted and inverted tooth pathology. The intervention of the healthcare professional should be guided by the clinical and imaging monitoring, with the medical-legal assessment in temporal and selective decisions of the therapeutics.
