Abstract: O cancro do ovário é a neoplasia ginecológica com maior mortalidade e a quinta causade morte em mulheres, sendo a sua taxa de incidência na população portuguesa de 8,2/100.000 mulheres. Os tumores do ovário são responsáveis por cerca de 3,7% dos óbitos napopulação feminina, em Portugal, com uma relação incidência/mortalidade global de 2:1. Aelevada letalidade desta neoplasia está relacionada com o facto de ser uma doença comsintomatologia inespecífica ou ausente em estádios iniciais, com o diagnóstico a serestabelecido quando a massa tumoral atinge grande volume ou está presente sob a forma decarcinomatose peritoneal. Considerando as manifestações clínicas inespecíficas e tardias, a falta de métodos efetivos de diagnóstico precoce e a grande dificuldade em diferenciar massas tumorais benignas e malignas nos estádios iniciais, este trabalho pretende fazer uma revisão às diferentes estratégias de diagnóstico de tumores do ovário. Para a realização deste artigo de foi efetuada uma pesquisa bibliográfica na base dedados PubMed acerca de estratégias de diagnóstico de massas anexiais validadas erecomendadas, sendo descritas neste artigo o Risk Of Malignancy Index, IOTA LogisticRegression model 2, IOTA Simple Rules, Assessment of Different NEoplasias in the adneXa(ADNEX), Risk of Ovarian Malignancy Algorithm (ROMA) e Subjective Assessment. Concluiu-se que, apesar de os modelos e algoritmos de cálculo do risco de malignidade não apresentarem uma especificidade superior à avaliação subjetiva por um ginecologista experiente, poderá ser útil aplicá-los e testá-los como teste de triagem numa estratégia de duas etapas, orientando o encaminhamento de mulheres com massas anexiais suspeitas para a avaliação ecográfica num centro de referência. Novas pesquisas devem explorar se estes métodos, isoladamente ou combinados com os biomarcadores recentemente investigados, podem servir como base de um programa de rastreio com potencial para evitar cirurgias desnecessárias sem atrasar o diagnóstico de cancro do ovário.
Ovarian cancer is the gynecological neoplasia with the highest mortality rate and thefifth leading cause of death in women, with an incidence rate of 8.2 per 100,000 in thePortuguese population. Ovarian tumors account for about 3.7% of deaths in the female population, in Portugal, with an overall incidence / mortality ratio of 2:1. The high lethality of this neoplasia is related to the fact that it is a disease with nonspecific or absent symptoms in the early stages, with the diagnosis to be established when the tumor mass reaches large volume or is present as peritoneal carcinomatosis. Considering the nonspecific and late clinical manifestations, the lack of effective methods of early diagnosis and the great difficulty in differentiating benign and malignant tumor masses in the early stages, this work intends to review the different strategies for the diagnosis of ovarian tumors. For the accomplishment of this article, a bibliographic search was performed in the PubMed database about validated and recommended adnexal mass diagnostic strategies. In this article are described the Risk Of Malignancy Index, IOTA Logistic Regression model 2, IOTA Simple Rules, Assessment of Different Neoplasms in the adnexa (ADNEX), Risk of Ovarian Malignancy Algorithm (ROMA) and Subjective Assessment. It was concluded that, although the models and algorithms for calculating the risk ofmalignancy do not present a higher specificity than the subjective assessment by anexperienced gynecologist, it may be useful to apply them as a screening test in a two-step strategy, referring women with suspicious adnexal masses for ultrasound evaluation by an experienced gynecologist. New research should explore whether these methods, alone or in combination with the recently investigated biomarkers, can provide the basis of a screening program that can prevent unnecessary surgeries without delaying the diagnosis of ovarian cancer.
