múltiplas dimensões que definem a sua história natural, morbilidade e mortalidade. A
sua classificação é feita em função da gravidade da limitação do débito aéreo. Contudo,
este parâmetro não reflecte a heterogeneidade da apresentação clínica desta patologia,
sendo então possível admitir que a actual definição da DPOC inclua diferentes grupos
(fenótipos clínicos) de doentes com características clínicas e/ou biológicas comuns, com
prognósticos e orientação terapêutica diferentes. Neste sentido foi elaborada uma
revisão na Pubmed, entre 1955 e 2012, limitada às línguas inglesa, portuguesa, francesa
e espanhola. A análise comparativa dos estudos disponíveis até à data permitiu definir
as características fenotípicas significativas neste contexto e distinguir 3 fenótipos –
misto, exacerbador frequente, enfisematoso - definidos por sintomas, fisiologia e
imagiologia. A caracterização fenotípica rigorosa é importante, e idealmente, o grupo de
indivíduos que partilha o mesmo fenótipo deveria ser determinado pelo mesmo
mecanismo biológico ou fisiológico, e desta forma, apresentar o mesmo perfil de
resposta à terapêutica. A ciência em rede e a proteómica poderão contribuir largamente
nesta área. O desafio actual consiste na identificação de fenótipos distintos que
constituam o síndrome DPOC e no estabelecimento de regras válidas e simples que
avaliem os outcomes e a resposta à terapêutica, de forma a avançar em direcção ao
modelo P4 (Personalized, Predictive, Preventive e Participatory) Chronic obstructive pulmonary disease (COPD) is a complex disease with multiple dimensions which defines its natural history, morbidity and mortality. The classification of airflow limitation severity in COPD does not reflect the heterogeneity of its clinical presentation and, consequently, within what is known today as COPD, there may be some groups (clinical phenotypes) of patients who share clinical and/or biological characteristics but who have distinct prognoses and/or who require different therapeutic approaches. The research was based on PubMed, between 1955 and 2012, and restricted to results published in English, Portuguese, French and Spanish. The comparative analysis allowed defining significant phenotypic characteristics and distinguishing 3 phenotypes – overlap, frequent exacerbator and enfisematous – defined by symptomatic, physiologic and imagiologic criteria. The characterization of phenotypic variation is important and, ideally, the group who share the same phenotype would be determined by the same biological mechanism, and therefore, presenting the same therapeutic profile. The network science and the proteomics are likely to largely contribute in this area. The actual challenge relies on identifying distinct phenotypes that make COPD syndrome and establishing simple and valid rules that evaluate outcomes and response to therapy, to progress towards the P4 model (Personalized, Predictive, Preventive e Participatory) |
