Possíveis estratégias para erradicar o HIV de todos os seus reservatórios no organismo humano: perspetivas de cura

Abstract

Apesar dos resultados favoráveis provenientes da utilização da terapêutica HAART, existem vários reservatórios virais, celulares e anatómicos, que podem contribuir para a persistência do HIV-1, especialmente os linfócitos T CD4+ de memória, em repouso e o tecido linfoide. Estes reservatórios constituem a principal barreira à eliminação do vírus do organismo humano e consequente cura da infeção. Nos últimos anos surgiram alguns casos intrigantes e inspiradores que indicam que uma cura total ou parcial da infeção é uma possibilidade, como por exemplo os casos “Berlin Patient’” ou “Mississipi Baby”, podendo acreditar-se que se trata de uma questão de tempo até encontrar uma cura para esta infeção. Esta monografia tem como objetivo conhecer os reservatórios virais existentes em doentes tratados e os mecanismos de estabelecimento e manutenção da latência viral, analisando possíveis estratégias para a eliminação do vírus dos seus reservatórios no organismo e possível cura da infeção por HIV.

In spite of the favorable results obtained by HAART therapy, several viral, cellular and anatomical reservoirs can contribute to HIV-1 persistency, specially CD4+ memory T cells and lymphoid tissue. These reservoirs are the main barrier to the elimination of the virus from the human organism and consequentially the cure of the infection. In the last few years, there have been some intriguing and inspiring cases that indicate that total or partial cure is a possibility, for example, "Berlin Patient" or "Mississippi Baby", making it possible to believe that finding a cure for this infection is only a matter of time. This monograph addresses the subject of viral reservoirs existing in treated patients and the establishment and maintenance mechanisms of viral latency, analyzing possible strategies for eradication of the virus from its human reservoirs and possible cure for HIV infection.
