Title: Estudo clínico e imunológico comparativo entre o lúpus eritematoso sistémico de início juvenil e do adulto

Abstract: Introdução O Lúpus Eritematoso Sistémico (LES) é uma doença reumática multissistémica auto imune, com largo e variado espectro clínico, associada à presença de anticorpos anti nucleares. Na maioria dos casos, a doença inicia-se na idade adulta jovem. Contudo, alguns casos podem manifestar-se em idade juvenil (≤ 18 anos). Existem poucos estudos prévios comparativos entre o LES de início no adulto e Juvenil e nenhum em Portugal. Estes trabalhos sugerem diferenças na apresentação clínica e prognóstico Objectivos Caracterizar e comparar as diferenças nas manifestações clínicas e perfil imunológico entre o LES de início juvenil e o LES de início no adulto. Doentes e Métodos Estudo retrospectivo de 230 doentes (167 de início da doença em idade adulta e 63 de início da doença em idade juvenil) seguidos no Hospital Pediátrico de Coimbra e na Consulta de Lúpus Eritematoso Sistémico do Serviço de Reumatologia dos Hospitais da Universidade de Coimbra. Todos os doentes cumpriram 4 ou mais critérios de classificação para LES do American College of Rheumatology. Os doentes serão classificados como LES juvenil nos casos com início das manifestações da doença até aos 18 anos de idade e como LES do adulto com manifestações iniciais após a mesma idade. Resultados Os doentes com LES de início juvenil apresentaram mais frequentemente manifestações neurológicas, eritema malar e trombocitopenia. Na prevalência das restantes manifestações clínicas estudadas não houve diferenças significativas entre os 2 grupos. O grupo com LES de início juvenil apresentou mais frequentemente Anti-RNP e Anti-Sm. Por outro lado o grupo com LES de início no adulto exibiu mais frequentemente Anti-dsDNA. Nos restantes autoanticorpos estudados não se verificaram diferenças significativas entre os grupos. Conclusões Este é o primeiro estudo Português comparando o LES de início juvenil (jLES) com o LES de início no adulto (aLES) quanto às manifestações clínicas e ao perfil imunológico. Neste estudo foi confirmado que há diferenças entre os grupos e o LES de início juvenil poderá apresentar um curso de doenças mais severo. Foi demonstrado que o jLES apresenta com maior frequência alterações neurológicas, trombocitopenia e eritema malar. Quanto ao perfil imunológico evidenciou-se que o jLES apresenta Anti-RNP e Anti-Sm com maior frequência e que o grupo aLES apresenta mais frequentemente Anti-dsDNA.
Introduction Systemic lupus erythematosus it’s a rheumatic multisystemic autoimmune disease, with a large and variable clinical specter, associated with the presence of antinuclear antibodies. In most cases, the disease begins in adulthood. However, some cases may occur in young age (≤ 18 years). There are few previous studies comparing the adult onset SLE and Juvenile and none in Portugal. These studies suggest differences in clinical presentation and prognosis. Objectives To characterize and compare the differences in clinical manifestations and immunological profile between the juvenile-onset SLE and adult onset SLE. Patients and Methods A retrospective study of 230 SLE patients (167 adult onset SLE and 63 of juvenile onset SLE) followed at the Hospital Pediátrico de Coimbra and Lupus Clinic of the Rheumatology Department of the Hospitais da Universidade de Coimbra. All patients fulfilled 4 or more classification criteria for SLE of the American College of Rheumatology. Patients will be classified as juvenile-onset SLE if the disease started up to 18 years of age and as adult onset SLE if onset is after that age. Results The juvenile-onset SLE patients had more frequent neurologic manifestations, malar rash and thrombocytopenia. There were no significant differences between the two groups in the prevalence of other clinical manifestations studied here. The group with juvenile-onset SLE had more frequently anti-RNP and anti-Sm. Moreover the group with adult onset SLE showed more frequently anti-dsDNA. There were no significant differences between the two groups in other autoantibodies studied here. Conclusions This is the first Portuguese study comparing the differences in clinical manifestations and immunological profile between the juvenile-onset SLE and adult onset SLE. This study confirmed that there are differences between the groups and juvenile-onset SLE may present a more severe course of disease. It was shown that jSLE most often presents neurological disorders, thrombocytopenia and malar rash. As for the immunological profile showed that the jSLE presents Anti-RNP and anti-Sm with greater frequency and that the group aSLE more often presents Anti-dsDNA.
