agressão hepática constante pode evoluir na fase crónica para cirrose, insuficiência hepática e
carcinoma hepatocelular. Declarada como epidemia silenciosa, as mais recentes estimativas
indicam que, no mundo, mais de 185 milhões de pessoas estejam cronicamente infectadas
com o vírus da hepatite C.
Até à data, o standard de tratamento consistia na utilização do interferão peguilado
e ribavirina. Em doentes infectados com o genótipo 1 do vírus pode ainda adicionar-se o
boceprevir ou telaprevir, os primeiros antivirais de acção directa a serem aprovados.
Contudo, estas terapêuticas mostram-se ineficazes em muitos doentes e originam efeitos
secundários severos.
Objectivo: A presente monografia aborda o avanço no tratamento da hepatite C
com a introdução de um novo fármaco de acção antiviral directa, o sofosbuvir. Este fármaco
revolucionário demonstra, em combinação com outras moléculas antivirais, elevada eficácia
entre os vários genótipos e bom perfil de segurança sem interacções farmacológicas
relevantes. Hepatitis C is a viral disease with slow evolution and generally without symptoms. The constant liver injury may progress to cirrhosis in chronic phase, liver failure and hepatocellular carcinoma. Declared as a silent epidemic, the latest estimates indicate that, worldwide, more than 185 million people are chronically infected with hepatitis C. At present, the standard of care consisting in the use of pegylated interferon and ribavirin. In patients infected with genotype 1 virus may still be added boceprevir or telaprevir, the first direct antiviral agents to be adopted. However, these therapies prove to be ineffective in many patients and cause severe side effects. Objective: This monograph discusses the advancement in the treatment of hepatitis C with the introduction of a new drug with direct antiviral action, sofosbuvir. This revolutionary drug demonstrates, in combination with other antiviral molecules, high efficiency among the various genotypes and good safety profile without significant drug interactions. |
