e a terceira causa mais frequente de morte relacionada com neoplasias malignas. É ainda o
maior responsável pela mortalidade de doentes cirróticos, ocorrendo em 80% dos casos neste
contexto. Os procedimentos diagnósticos e terapêuticos que lhe estão associados estão cada
vezes mais estandardizados constituindo guidelines subscritos pelas mais importantes
associações internacionais para Estudo do Fígado.
Objectivos
Pretende-se a criação de uma base de dados sobre carcinoma hepatocelular, onde esteja
reunida todas as informações essenciais, permitindo um seguimento mais sistematizado destes
doentes. Expõem-se também informações de doentes que foram relevantes para a sua
constituição.
Metodologia
Fez-se uma análise do panorama actual do conhecimento do hepatocarcinoma na literatura
científica e recolheram-se os dados de um grupo de 14 doentes de forma a definir as
informações que deveram estar presentes na base de dados. Fazendo uso do programa
FileMaker®, que permite criação de bases de dados podendo-se proceder a modificações sem
perda de dados antigos, criou-se um conjunto de layouts e adicionaram-se os dados dos
referidos doentes.
Resultados
Criou-se assim uma base de dados constituída por layouts dedicados ao hepatocarcinoma.
Depois da identificação do doente envolvido colhem-se dados sobre o método que permitiu a
descoberta da neoplasia. As informações necessárias para o estadiamento proposto pelo grupo
de Barcelona (BCLC) designadamente o performance status, o Child-Pugh, os dados
fundamentais da morfologia e da localização do tumor bem como elementos sobre a invasão
vascular ou a presença de metástases são recolhidos de modo sistemático. Anotam-se
igualmente dados definidores da hepatopatia subjacente.
As modalidades terapêuticas possíveis são depois dissecadas. Assim existe um layout para
eventuais ressecções cirúrgicas; um outro para transplante hepático e suas complicações;
ainda um outro para os procedimentos ablativos percutâneos. Para uma eventual
quimoembolização bem como para a utilização de inibidores das quinases existem layouts
com campos apropriados que permitem recolher dados sobre as eficácia e os correspondentes
efeitos adversos. Termina-se com um layout dedicado ao seguimento geral dos doentes. Nela
foram implementadas as informações dos doentes que serviram para a melhorar e adaptar à
realidade do serviço hospitalar.
Conclusão
Conseguiu-se com este trabalho fazer uma base de dados sobre carcinoma hepatocelular que
pretende tornar o seu seguimento mais sistematizado e as decisões mais correctas. Fez-se
ainda uma casuística dos doentes cujas informações contribuíram para a formação da base de
dados. Hepatocellular Carcinoma is the sixth most frequent neoplasia in the world and the third cause of death from malignant neoplasia. It is also the number one cause of death in cirrhotic patients, occurring in 80% of these patients. The associated diagnosis and therapeutic procedures are becoming more standardized and constitute guidelines endorsed by the most important international associations for the Study of Liver. The objective of this work is to create a database on hepatocellular carcinoma, in which are assembled all of the essential data of this tumor, allowing a systematized follow-up of these patients. It also disclosures relevant information from patients used in the database constitution. An analysis from the current knowledge of hepatocellular carcinoma in the scientific literature was conducted and data was collected from 14 patients, in order to define the relevant information to be included in the database. Making use of the Filemaker® software, that allows the creation of databases and their modification without loss of previous data, it was created a group of layouts and added the information of the patients. In this manner, we created a database formed by several layouts dedicated to hepatocellular carcinoma. After the identification of the patient, we collect information on the method that enabled the discovery of the tumor. Using a systematized methodology, we collected the data needed for staging with the Barcelona Clinic Liver Cancer classification, namely performance status, Child-Pugh, crucial morphological and location specifics of the tumor, as well as vascular invasion and metastatic disease. It is also registered the defining information of underlying liver disease. The therapeutic modalities are then described. Accordingly, there is a layout for surgery, another one for liver transplantation and its complications and another one for ablative procedures. The transarterial chemoembolization as well as the use of the multikinase inhibitor, Sorafenib, have data fields about their effectiveness and complications in the corresponding layouts. The final layout is designated to the patient general follow up. In the database, we implemented patient information that helped to improve the database itself and also to adapt it to the reality of hospital service In conclusion, we created a database on hepatocellular carcinoma with the objective of a uniform follow up of the patients and to contribute for more correct therapeutic decisions. We also analyzed the data of the patients whose information contributed to the development of this database. |
