Abstract: Introdução A Miocardiopatia Hipertrófica (MCH) é uma doença primária do sarcómero cardíaco, que se caracteriza por hipertrofia ventricular esquerda (VE), sem dilatação da cavidade ventricular e com envolvimento ocasional do ventrículo direito. Dada a heterogeneidade de expressão fenotípica, bem como o seu curso clínico variável, a abordagem diagnóstica da MCH permanece questionável. Apesar de a ecocardiografia transtorácica constituir a técnica de imagem de primeira linha no diagnóstico da MCH, a Ressonância Magnética Cardíaca (RMC) permite uma avaliação morfológica e funcional do coração, não reprodutível por outro método não invasivo. Objectivos Constituíram objectivos do presente estudo: 1) Descrever alterações encontradas na RMC de uma população de doentes seguidos nos Hospitais da Universidade de Coimbra (HUC), com diagnóstico confirmado de MCH. 2) Determinar se a RMC permite maior acuidade que a ecocardiografia transtorácica em estabelecer o diagnóstico de MCH e aceder à magnitude de hipertrofia VE. 3) Analisar a relação entre a presença de realce tardio (RT) na RMC, e as características clínicas e demográficas da população em estudo. Metodologia Foram identificados 12 doentes, com diagnóstico confirmado de MCH, que realizaram RMC nos Hospitais da Universidade de Coimbra (HUC), no período de tempo compreendido entre 01/06/2006 e 28/10/2010. Realizou-se uma análise descritiva e analítica das variáveis demográficas e clínicas da população em estudo, bem como das alterações imagiológicas. Diana Rita da Cruz Martins 4 Procedeu-se a uma análise comparativa entre os dados previamente obtidos na ecocardiografia transtorácica e os referentes à RMC, avaliando ainda a existência de associação significativa entre a identificação de RT na RMC e as variáveis demográficas, clínicas, bem como factores de risco para morte súbita cardíaca (MSC). Resultados Na população em estudo, identificou-se igualdade de géneros, apresentando uma idade média de 37±23 anos. Verificou-se que os valores de espessura máxima da parede VE, mensurados por ecocardiografia e RMC, foram estatisticamente diferentes. Identificaram-se áreas de RT em 91,7% dos doentes porém, não foram obtidos resultados estatisticamente significativos quando relacionado o RT com dados demográficos e clínicos da população em estudo. Conclusões A RMC identificou valores estatisticamente superiores de hipertrofia VE, quando comparada com a ecocardiografia transtorácica, reconhecendo igualmente doentes que apresentavam graus de hipertrofia massivos (espessura máxima ≥ 30mm), colocando-os assim num subgrupo de alto risco. Não foi identificado valor prognóstico associado à presença de RT na RMC.
Introduction Hypertrophic Cardiomyopathy (HCM) is a primary disease of the cardiac sarcomere, characterized by left ventricular hypertrophy, without ventricular cavity dilatation and with occasional involvement of the right ventricle. Owing to its heterogeneity of phenotypic expression and variable clinical course, the diagnostic approach of HCM remains questionable. Despite the fact that transthoracic echocardiography still remains the first line imaging choice for the diagnosis of HCM, the Cardiovascular Magnetic Resonance (CMR) allows a functional and morphologic cardiac evaluation, that’s not reproducible by other non invasive diagnostic procedure. Objectives The main objectives of this study were: 1) Describe the alterations found in a population of patients imaged with CMR, at Coimbra’s University Hospital (HUC). 2) Define if CMR has more accuracy than echocardiography in the diagnosis of HCM, and in the determination of the magnitude of hypertrophy. 3) Analyze the relation between the occurrence of late gadolinium enhancement (LGE) in CMR, and the clinical and demographic characteristics of the study population. Methods Twelve patients with confirmed diagnosis of HCM, that performed CMR at HUC between 1st-June-2006 and 28th-October-2010, were selected. It was performed a descriptive and analytic examination of the demographic, clinical and imagiological characteristics of the study population. It was made a comparative analysis between the previous echocardiographic Diana Rita da Cruz Martins 6 information and the CMR assessment, and it was also evaluated if there was significant association between the identification of LGE in CMR and the demographic and clinical characteristics of the patients, as well as risk factors of sudden cardiac death (SCD). Results The study population was aged between 12 and 77 (37 ± 23) years, and 50% were male. The values of left maximal ventricular thickness, measured by echocardiography and CMR, were statistically different. LGE was identified in 91,7% of patients however, there weren’t obtained statistically significant results when it was correlated with demographic and clinical characteristics of the population. Conclusions The CMR identified superior values of left ventricular hypertrophy, when compared with transthoracic echocardiography, that also underestimated massive hypertrophy degrees, which were recognized by CMR, setting those patients in an high risk level subgroup. However, it wasn’t identified prognostic value associated with LGE identification
