Title: A infertilidade em medicina geral e familiar : a abordagem e o seguimento do casal

Abstract: Introdução: A infertilidade é um problema de saúde com grande impacto biopsicossocial. O médico de família é o profissional de saúde mais próximo do casal infértil, estando numa posição privilegiada para dar início à abordagem da infertilidade. Métodos: Com base na apresentação de um caso clínico, o objetivo deste artigo é apresentar uma revisão teórica da problemática da infertilidade sob o ponto de vista da Medicina Geral e Familiar. São abordados o diagnóstico, a investigação etiológica, as alternativas terapêuticas e a importância do seguimento do casal ao longo de todo o processo. Resultados: A infertilidade ronda os 14% na Europa, afetando um em cada sete casais. Em Portugal cerca de 8,9% das mulheres entre os 25 e os 44 anos foram diagnosticadas como inférteis. Apesar de apenas 26% dos casais inférteis portugueses recorrerem ao médico de família, este clínico diagnostica e investiga fatores de risco reprodutivo e causas de infertilidade. O tratamento de algumas situações pode estar ao alcance da Medicina Geral e Familiar e quando tal não se verifica é da sua competência referenciar o casal. Para além disso, deve caber a este clínico a orientação do casal durante todo o processo. Conclusões: De forma a incrementar o papel deste clínico, é necessário incentivar o uso das linhas de orientação existentes e apostar em formação diferenciada para que os médicos de família se sintam mais confiantes na abordagem do casal infértil
Background: Infertility is a health problem with great biopsychosocial impact. General practitioners are the health professionals closest to the infertile couple, being in a privileged position to start the investigation of infertility. Methods: Based on the presentation of a case, the objective of this paper is to present a bibliographical review about infertility, according to general practitioner’s approach. We address the diagnosis, etiology research, treatments and the importance of the following the infertile couple through the process. Results: In Europe, infertility is around 14%, affecting one in seven couples. In Portugal, about 8.9% of women between 25 and 44 years were diagnosed as infertile. Although only 26% of Portuguese infertile couples resort to general practitioners, this clinician is able to diagnose and investigate reproductive risk factors and causes of infertility. Treatment of some situations can be within reach of General Practice and when such is not the case is his competence to refer the couple. Furthermore, general practitioner is able to do couple’s follow-up throughout the process. Conclusions: In order to enhance the role of the clinician, it is necessary to encourage the use of existing guidelines and invest on differentiated training in order to the general practitioner feel more confident in the approach of the infertile couple.
