Title: Infeções do SNC na infeção por VIH/SIDA

Abstract: O neurotropismo do VIH é um dos principais problemas associados à infeção por VIH/SIDA. A consequência desta infeção é devastadora, uma vez que induz inflamação crónica. As infeções oportunistas do SNC surgem, então, neste contexto como uma manifestação com alta mortalidade e mau prognóstico. Desde o início da TARc, em 1996, o diagnóstico de infeção VIH deixou de ser uma sentença de morte para passar a uma doença crónica, passível de controlo. Este artigo pretende analisar quatro infeções oportunistas do SNC, a LMP, a toxoplasmose, a tuberculose e a meningite criptocócica e enquadrá-las numa perspetiva pré-TARc vs após-TARc. O tema foi revisto utilizando a interface de pesquisa da biblioteca de conhecimento online - Elsevier e PubMed da Medline, com as palavras-chave inglesas: “HIV infection”, “Opportunistic infections in CNS”, “IRIS” e “HAART in CNS”, cujos resultados foram restringidos, maioritariamente, a artigos em inglês, publicados desde o ano 2010 até à atualidade, os quais foram selecionados pela sua relevância. Adicionalmente foram consultados artigos do website UpToDate® e o Harrison’s Principals of Internal Medicine 19th Edition, para além de todas as normas do programa nacional para a infeção VIH/SIDA, inserido na Direção Geral da Saúde. Os vários estudos analisados eram consensuais ao afirmar uma diminuição significativa na incidência de infeções oportunistas do SNC desde o início da TARc, exceto para a LMP, que se manteve estável. A TARc melhorou, também, o prognóstico destes doentes, mostrando a eficácia desta terapia, a qual pode, contudo, ser perturbada por vários motivos, sendo que a adesão à terapêutica apresenta uma contribuição significativa. Por outro lado, vários casos de diagnóstico de infeção VIH tiveram como apresentação inicial uma infeção oportunista do SNC, nomeadamente a toxoplasmose. Assim, a eficácia da TARc ficou comprovada pela diminuição da incidência, contudo, a adesão à terapêutica continua a ser um grande problema que leva a muitas descontinuações da terapia antirretroviral, com todas as consequências que acarreta. Reveste- -se, assim, de grande valor conhecer as características das quatro infeções apresentadas, uma vez que devem ser colocadas como diagnóstico diferencial em momentos em que a suspeita e/ou o quadro clínico sejam sugestivos. The HIV neurotropism is one of the main problems associated with HIV / AIDS. The consequences are devastating, as it induces chronic inflammation. Opportunistic CNS infections arise in this context as a manifestation with high mortality and poor prognosis. Since the beginning of ART in 1996, the diagnosis of HIV infection is no longer a death sentence to turn out to be a chronic disease, which this treatment can control. This article analyses four opportunistic infections of the CNS, PML, toxoplasmosis, tuberculosis and Cryptococcal meningitis, and read them in the pre-ART vs. ART perspective. The subject has been reviewed using the online knowledge library search interface - Elsevier and PubMed Medline, using the key words in the English language, "HIV infection", "Opportunistic infections in CNS," IRIS "and" HAART in CNS " . The results of the research were mainly restricted to English language articles published from 2010 to the present. Articles were selected for the relevance of its content. Additionally, items were consulted in the website UpToDate® and Harrison's Principals of Internal Medicine 19th Edition. All the guidelines of the national program for HIV / AIDS infection inserted into the Direção Geral da Saúde were consulted. Several studies agree by stating a significant decrease in the incidence of opportunistic infections of the CNS since the start of ART, except for PML, which remained stabilized. The ART improved, also the prognosis of these patients, showing the effectiveness of this therapy. This efficacy may, however, be disrupted by various reasons, and adherence to therapy provides a significant contribution. Moreover, many patients diagnosed with HIV infection were initially presented as an opportunistic infection in the CNS, particularly toxoplasmosis. Thus, the effectiveness of ART was evidenced by decreased incidence; however, adherence to treatment remains a major problem that leads to many discontinuations of antiretroviral therapy, with all the consequences that entails. In these cases, it is important carry out joint and continuous efforts. It is, also, very important to know the characteristics of the four selected infections, since they are included in the differential diagnosis whenever the clinical picture is suggestive.
