Aguda (LMA) caracterizada por alterações genéticas específicas, envolvendo
tipicamente rearranjos no gene do Recetor Alfa do Ácido Retinóico (RARA),
responsáveis pela acumulação de promielócitos leucémicos na medula óssea e sangue
periférico - ocasionando, frequentemente, uma coagulopatia de consumo potencialmente
fatal.
A maioria dos casos de LPA caracteriza-se pela fusão do gene PML, no
cromossoma 15, com o gene RARA, localizado no cromossoma 17 – embora
ocasionalmente anomalias citogenéticas alternativas possam encontrar-se presentes. O
RNA mensageiro da fusão PML-RARA codifica uma proteína quimérica que
compromete o processo de diferenciação celular na fase promielocítica. O conceito
chave que justifica a abordagem terapêutica promissora atual assenta na elevada
sensibilidade terapêutica da oncoproteína de fusão PML-RARA ao Ácido Trans-
Retinóico (ATRA), com consequente retoma do processo diferenciação granulocítica.
O tratamento atual da LPA consiste na administração de ATRA e quimioterapia
com antraciclinas, abordagem terapêutica altamente eficaz na indução da remissão.
Consequentemente, a LPA converteu-se numa das LMA mais potencialmente tratáveis,
apresentando tipicamente um excelente prognóstico. O aperfeiçoamento posterior dos
regimes terapêuticos com ATRA e Trióxido de Arsénio (ATO) ofereceu a cura à
maioria dos doentes com LPA. A combinação terapêutica livre de quimioterapia
revelou-se inovadora ao provar-se altamente eficaz e potencialmente curativa em muitos
doentes - uma descoberta excitante enquanto abordagem terapêutica altamente
promissora.
No entanto, apesar da atualização dos regimes terapêuticos ao longo dos últimos
anos ter proporcionado enormes progressos no que concerne ao prognóstico da LPA,
existe ainda um número significativo de doentes que privam da remissão da doença por
mortalidade precoce. Embora a resistência primária à terapêutica tenha já sido
contornada em grande escala com os protocolos terapêuticos atuais, a mortalidade
precoce por um quadro de diátese hemorrágica no estádio inicial da doença, revela-se
um importante obstáculo à cura da LPA.
O quadro de coagulopatia envolve habitualmente hemorragia grave de órgãos
vitais, como o sistema nervoso central, pulmões e trato gastrointestinal; podendo,
inclusive, motivar quadros trombóticos. A LPA é uma das doenças hematológicas cujo
reconhecimento precoce e o início imediato da terapêutica dirigida à primeira suspeita
de doença, antes da confirmação citogenética ou molecular do diagnóstico,
concomitante com a terapêutica de suporte agressivo, se revelam fundamentais na
redução da mortalidade precoce. Os doentes que sobrevivem aos primeiros 30 dias de
doença, período em que a coagulopatia e a síndrome de diferenciação pelo ATRA
condicionam elevado risco de morbilidade e mortalidade, apresentam uma possibilidade
de cura extremamente elevada.
O objetivo desta revisão é fazer um estado da arte das publicações e dados mais
recentes relacionados com esta doença hemato-oncológica, incluindo compreender o
papel dos mecanismos fisiopatológicos, apresentar as características clínicas, discutir a
conduta e realçar as opções terapêuticas atualmente aprovadas e ainda em investigação. Acute Promyelocytic Leukemia is a distinct subtype of acute myeloid leukemia characterized by a specific genetic alteration, typically involving retinoic acid receptor alpha (RARA) gene rearrangements, that leads to the accumulation of leukemic promyelocytes in the bone marrow and blood which is frequently associated with a lifethreatening consumptive coagulopathy. Most cases of APL contain the fusion of the promyelocytic leukemia (PML) gene on chromosome 15 with the RARA gene on chromosome 17, although occasional cases have other variant cytogenetic abnormalities. The fusion messenger RNA PML-RARA encodes a chimeric protein that impairs normal differentiation at the promyelocyte stage. The PML-RARA fusion gene product is specifically targeted by the drug all-trans retinoic acid, which promotes granulocytic differentiation - basic concept that supports the current promising therapeutic approach. The current standard treatment given to patients with newly diagnosed APL consists of all-trans retinoic acid (ATRA) and anthracycline-based cytotoxic chemotherapy, which is highly effective for remission induction. As a result, APL is considered one of the most treatable AMLs and typically has an excellent prognosis. Subsequent refinement of ATRA treatment schedules and the introduction of arsenic trioxide (ATO) allow the cure of the majority of patients with APL. The groundbreaking chemotherpay-free combination of ATRA and ATO is a highly effective and potentially curative treatment - in many, if not most, patients - and this has now emerged as the most exciting new treatment strategy. However, even though timely intervention of ATRA and ATO has made enormous progress in the prognosis of APL, there are still a fair number of patients deprived of the chance to remission by early death. While primary leukemia resistance has virtually disappeared as a cause of remission induction failure, death due to hemorrhage remains the major problem during the early treatment phase. Severe coagulopathy usually results in an outbreak of hemorrhage in vital organs, most frequently occurring in the central nervous system, the lung and the gastrointestinal tract, and as well as the frequent incidence of thrombosis. APL is one of the few hematologic diseases that the early recognition and prompt initiation of treatment at first suspicion remains a challenge in clinical practice, once rapid institution of ATRA at the first suspicion of the disease before cytogenetic or molecular confirmation of the diagnosis and aggressive blood product support are critical to reduce early mortality. Patients who survive the first 30 days after diagnosis of their disease, when the coagulopathy and the ATRA differentiation syndrome can cause morbidity and mortality, have an extraordinarily high rate of cure. The aim of this review is to make a state of art of the most recent available data concerning this hemato-oncological disease, including understand the role of pathophysiology mechanisms, introduce the clinical features, discuss the management and highlight the therapeutic options already approved and that are still under investigation. |
