Experiências de adversidade precoce e sintomas medicamente não explicados : o caso particular da fibromialgia

Abstract

Vários estudos têm tornado aparente de como as experiências de adversidade precoce podem influenciar a vida das suas vítimas, nomeadamente no que diz respeito às suas consequências a nível da saúde tanto física como mental dos indivíduos. O objetivo deste trabalho é o de efetuar uma revisão bibliográfica, aprofundada e atualizada, relativamente às experiências traumáticas de adversidade precoce, investigando, sobretudo, até que ponto poderão relacionar-se mais tarde com o desenvolvimento de síndromes somáticas funcionais em geral, e de fibromialgia em particular. As implicações clínicas de tal relação são, também, exploradas. Foi realizada uma pesquisa na PubMed, MEDLINE e PsycINFO para obtenção de material bibliográfico publicado entre 1990 e 2012. Deu-se especial relevância às publicações mais recentes, pertencentes a revistas científicas de maior impacto e provenientes de autores de referência. Apesar das fortes evidências a favor de uma relação entre o abuso infantil e o desenvolvimento de várias síndromes dolorosas no adulto, esta relação tem sido frequentemente questionada sobretudo devido a várias questões de ordem metodológica dos estudos elaborados que não permitem obter conclusões definitivas. Também não existe consenso absoluto sobre se a revelação de experiências de abuso por parte destes doentes trará ou não benefícios a nível da sua abordagem terapêutica. Em todo o caso, o clínico deve ter conhecimento da forma de abordar corretamente estas questões com os seus pacientes. Sugere-se que mais investigações sejam elaboradas até que surjam soluções mais definitivas de forma a otimizar a ajuda a prestar a estes doentes e ao alívio do seu sofrimento. Salienta-se também a importância da identificação e abordagem precoce das crianças vitimizadas, com o objetivo de lhes oferecer a oportunidade de se submeterem a estratégias interventivas que visem a amenização dos efeitos adversos dos maus-tratos. Torna-se assim possível atuar a título preventivo, numa atitude proativa que tem o potencial de trazer benefícios significativos ao desenvolvimento destas crianças.

Several studies have shown how adverse childhood experiences may influence the life of their victims, namely in what concerns its consequences on the individual’s physical and mental health. The objective of this study is to undertake an in-depth and updated bibliographic review of the literature on adverse childhood experiences, investigating, mainly, the extent to which it can be related with the future development of functional somatic syndromes in general and, in particular, fibromyalgia. The clinical implications of this relationship are also explored. PubMed, MEDLINE and PsycINFO databases were searched to retrieve bibliographic material published between 1990 and 2012. Special relevance was given to more recent publications, namely those belonging to scientific journals with higher impact, and those from key authors in the field. In spite of strong evidence favoring a relationship between child abuse and the development of many painful syndromes in adulthood, this relationship has been frequently challenged mainly due to several methodological issues which do not enable researchers to obtain definitive conclusions. There is also no consensus on whether or not the revelation of abuse experienced by these patients has any benefit on their therapeutic approach. In any case, the caregiver must know how to correctly approach these issues with their patients. It is suggested that further research be undertaken until more definitive solutions are found to optimize the clinical help provided to these patients and minimize their pain. It is also important to highlight the importance of identifying and dealing with child abuse early on, in order to provide children with the chance to undergo interventional strategies that seek to reduce the adverse effects caused by the abuse. Thus, it is possible to act in a preventive manner, adopting a pro-active attitude which has the potential to provide significant benefits to the development of these children.
