da mama não é consensual, sendo a sua capacidade em avaliar o tamanho, a multifocalidade e
a multicentricidade de um tumor alvo de discussão no presente. Este estudo pretende
estabelecer um paralelismo entre a realidade no Centro Hospitalar e Universitário de Coimbra
(CHUC) e a literatura actual.
Materiais e Métodos: foram seleccionados, retrospectivamente, os doentes que realizaram
RM no Serviço de Imagiologia do CHUC entre Janeiro de 2012 e Janeiro de 2013 para
estadiamento local de cancro da mama recentemente diagnosticado. Foram recolhidos dos
“Processos Únicos” das doentes vários parâmetros relativos à população, à RM, bem como
relativos à cirurgia e peça operatória e posteriormente organizados numa base-de-dados em
Microsft Excel ®. A análise estatística foi realizada com recurso ao software IBM® SPSS
Statistics, versão 20.0.
Resultados: Das 12 doentes seleccionadas para estudo, a RM detectou focos adicionais de
doença ipsi-laterais em 41,66% das mesmas, sendo detectada multifocalidade em 25% e
multicentricidade em 33,3%. Em nenhuma doente se verificou, pela RM, invasão do músculo
peitoral ou lesões contra-laterais. A correlação entre as dimensões da lesão principal na RM e
na peça cirúrgica não apresenta diferença estatística, com p = 0,207, no entanto o seu poder
estatístico é de apenas 0,452 (1-ß). Comparando o resultado da RM com a peça cirúrgica, em
termos de multifocalidade não há concordância significativa (p = 1,000) e em relação a focos
multicêntricos existe concordância significativa (p = 0,005; kappa = 0,800). Neste estudo
foram obtidos 2 falsos negativos e 1 falso positivo.

Discussão/Conclusão: Os parâmetros clínicos observados nas doentes do CHUC não se
coordenam totalmente com o descrito na literatura. Os resultados obtidos pela RM e a sua
posterior comparação com a peça podem ter sido influenciados pelo reduzido número de
doentes estudadas. No entanto deve ser tida em conta alguma falta de informação nos
processos únicos ou desvalorização indevida. É perceptível a concordância na relação entre
multicentricidade do tumor na RM e no estudo anatomo-patológico, bem como se torna
plausível um importante valor preditivo negativo da RM no estadiamento local do cancro da
mama Introduction: The use of Magnetic Resonance Imaging (MRI) for breast cancer local staging is not consensual, being its capability of evaluating tumour’s size, multifocality and multicentricity a major topic of discussion nowadays. The present study aims to establish a parallelism between the reality in Centro Hospitalar e Universitário de Coimbra (CHUC) and the current literature. Materials and methods: Patients that have been submitted to a MRI for local staging of their newly diagnosed breast cancer, in the Radiology Department of CHUC, between January 2012 and January 2013 were selected. Various parameters related to the population, MRI and to the surgery and surgical samples were collected from the patient’s individual file and posteriorly organised into a Microsoft Excel® database. The statistical analysis was carried out using IBM® SPSS Statistics software, version 20.0. Results: From the 12 patients selected for this study, MRI detected additional ipsilateral focus of disease in 41.66% of them, being identified multifocality in 25% and multicenticity in 33.3% of the cases. Contralateral lesions or pectoralis muscle infiltration were not verified in any case, using MRI. The correlation between dimensions of the principal lesion in the MRI and in the surgical sample is not statistically different, showing p=0.207, although its statistical power is only 0.452 (1-ß). Comparing the results of the MR with the surgical sample, there is no significant agreement in terms of multifocality (p = 1.000) but there is significant agreement relative to multicentric focus (p = 0.005; kappa = 0.800). In this study two false negatives and one false positive results were obtained. Discussion/conclusion: The clinical parameters observed in the CHUC patients are not in total agreement with the ones described in the literature. The results obtained through the 5 MRI and their further comparison with the surgical samples might have been influenced by the small number of patients studied. However some lack of information in the patient’s individual file and incidental misappreciation should be taken in consideration. There is a clear concordance in the relation between tumour’s multicentricity in the MRI and in the anatomopathological study, as well as an important negative predictive value of the MRI in the local staging of breast cancer was concluded |
